It did not add a new PvE map or Lethal difficulty: those arrived in Patch 4.0 on October 17, 2024. The distinction matters because the original headline combines features from two separate updates.
What Patch 3.0 added
Focus Entertainment announced Patch 3.0 as live on September 26, 2024, describing it as “packed with new features.” Its headline additions were ultrawide support on PC and private Operations. The full feature list also included a sparring arena aboard the Battle Barge, along with a custom-colour preset reset option and other gameplay, balance and campaign adjustments. Focus Entertainment’s Patch 3.0 notes contain the complete list.
Private Operations
Private Operations let players run a mission alone with bots or with one friend, without matchmaking with random players during the mission. This was a way to play Operations without filling the squad through public matchmaking.
Battle Barge sparring arena
The new arena gave squadmates a place to spar with weapons and grenades aboard the Battle Barge. It was accessible from Operations or Eternal War.

Did Patch 3.0 add a PvE map or Lethal difficulty?
No. The new Termination Operation, set on the planet Kadaku, and Lethal difficulty for Operations were introduced in Patch 4.0, announced October 17, 2024. They are not Patch 3.0 content. Focus’s Patch 4.0 notes also describe changes to melee archetypes and perks, enemy spawns, and Ruthless difficulty.

Termination added a new Operation; Lethal added a difficulty option for Operations. These were substantial PvE additions, but they came in the October update, rather than the September Patch 3.0 release.
Patch 4.1 followed with a hotfix
In its October 31, 2024 community update, Focus said Termination and Lethal had been released that month. The publisher also acknowledged that the content and adjustments had not met everyone’s expectations and pointed to Patch 4.1 as a hotfix with gameplay and balancing tweaks to weapons, difficulty and AI.
