The main disadvantages of online games are displacement and exposure. Long or badly timed sessions can push out sleep, physical activity, schoolwork and face-to-face time, and online spaces can expose players to harmful content, abuse and exploitation. A minority of young people also show signs of problematic gaming. These are risks rather than guaranteed outcomes. How much harm occurs depends on how much someone plays, when they play, what they play, and what else is going on in their life.
Time that displaces sleep, activity and people
The most consistent concern in public-health guidance is not gaming as such but time that takes the place of things a young person needs. The US Centers for Disease Control and Prevention, in its “Digital Behaviors and Health” guidance (page dated September 8, 2026), links heavy or poorly timed digital media use to several problems: disrupted sleep, lower mood, reduced physical activity, and difficulty concentrating at school. The risk is highest when screen use happens at night or replaces sleep, physical activity and in-person connection. That guidance covers digital media broadly, so it applies to online games but is not a finding about games alone.
Why timing matters as much as length
A two-hour session in the early evening that ends well before bed is a different situation from a two-hour session that runs past midnight. Late sessions are the clearest route from gaming to lost sleep, and lost sleep is the route most directly connected to the school and mood problems that parents and teachers report. Timing is therefore worth checking before total hours.
Problematic gaming: how common it is
The WHO Regional Office for Europe’s September 25, 2024 release, “Teens, screens and mental health,” summarises findings from the 2022 Health Behaviour in School-aged Children (HBSC) survey. It reports that 12% of adolescents were at risk of problematic gaming. The figure covers 11-, 13- and 15-year-olds in 44 countries and regions across Europe, central Asia and Canada. In the same survey, 34% of adolescents played digital games daily, and 22% played for at least four hours on days when they gamed.

These numbers describe risk on a screening measure. They are not a diagnosis, they do not describe every young person who plays a lot, and they do not identify a safe daily limit. The sources reviewed do not establish a universal threshold of game time that is harmless or harmful, so any figure of that kind should be treated with suspicion.
Harmful interactions in online spaces
Online play carries social risks that offline play usually does not. WHO’s overview “Digital environments and mental health” describes digital spaces as supporting connection, learning and access to services, while also creating exposure to harmful content, online violence, exploitation, cyberbullying and isolation. The same environment can therefore produce friendships and harassment. Players who spend long hours in voice chat or community spaces have more contact with those risks, but the sources do not quantify how often they occur in games specifically.

Academic performance
A 2019 systematic review and meta-analysis published in JAMA Pediatrics pooled 58 cross-sectional studies. Its pooled analysis found no association between overall screen-media time and academic performance. Video-game playing, however, was inversely associated with composite academic scores, meaning that more gaming went with lower scores in the studies analysed. A subgroup analysis found this association among adolescents but not among children.
Because the studies were cross-sectional, they capture a snapshot of play and grades at one point. They cannot show that gaming caused the difference. Students who game more may differ in sleep, study habits, family circumstances or existing difficulties, and any of these could account for part of the gap.

How strong is each concern?
The concerns above do not rest on equally strong evidence. The table separates what each source type can and cannot show.

Mixed evidence and the direction of cause
WHO Europe’s policy brief “Addressing the digital determinants of youth mental health and well-being,” published May 23, 2025, describes the evidence on digital technology and young people’s well-being as mixed. Some online activities can have positive and negative effects at the same time. The brief also describes a two-way relationship: heavier screen use may worsen mental-health problems, and existing problems may lead young people to use screens more. Both directions can operate at once, which makes a simple cause-and-effect story unreliable.

WHO Europe’s regional director, Dr Hans Henri P. Kluge, put the balance this way in a statement about social media: “It’s clear that social media can have both positive and negative consequences on the health and well-being of adolescents.” The remark concerns social media rather than games, but it captures the same pattern.
The CDC’s 2025 study of US teenagers found that high daily screen time was associated with poorer outcomes in several areas, including mental health and perceived support. Its authors caution that the cross-sectional design cannot show causality or direction, and that the type, content and quality of screen use were not accounted for. Findings of this kind should be read as a warning about screen time in general, not as proof that a particular game harms a player.

Questions to ask about your own play
If you are weighing one gaming routine against another, or deciding whether a habit has become a problem, these are the useful axes to examine:
- Session length: how long a typical session runs, and whether sessions tend to stretch longer than planned.
- Time of day: whether play happens late enough to cut into sleep.
- Displacement: whether gaming is replacing sleep, physical activity, schoolwork or in-person relationships.
- Communication and content: whether the game includes open voice or text chat with strangers, and what kind of content it contains.
- Effects on mood and school: whether sleep, mood or concentration at school are getting worse over time.
No ranking of game genres or platforms by harm is established in these sources, so the questions matter more than the title of the game.
Practical steps that reduce the risk
CDC guidance treats the quality of digital interactions as seriously as the amount of time spent. The following steps follow its general family strategies. They are good digital-wellbeing practice rather than a clinically proven treatment for gaming problems.
- Keep consoles and devices out of bedrooms, especially overnight.
- Set a play window that ends well before bedtime, and stick to it on school nights.
- Model the habits you want, including putting your own device away at meals and during family time.
- Agree on family rules for online behaviour: who can be contacted, what personal information stays private, and what to do when someone is abusive.
- Schedule in-person activity and friendships on a fixed basis so that play does not fill every free hour.
- Ask schools about screen-free options and encourage in-person interaction in clubs, sport and class activities.
If sleep, mood or schoolwork decline while play stays heavy, a conversation with a doctor or school counsellor is a reasonable next step. The sources reviewed here do not give a clinical pathway for diagnosing gaming problems, so that conversation should rely on the person’s overall functioning rather than a single hour count.

Online games are not inherently harmful, and the evidence does not support saying they inevitably cause mental illness, aggression, poor grades or social isolation. The disadvantages are concrete: lost sleep, displaced activity, exposure to harmful interactions, and a minority of players who struggle with control. Those are the points to manage.
