In The Binding of Isaac: Rebirth, Death is a Harbinger boss that attacks with scythes. The Rebirth Wiki lists Death at 450 health and lists Ball of Bandages and Cube of Meat among its drops. It is separate from Isaac’s player-death mechanic and the XIII – Death tarot card.
Where Death appears
The community-maintained Rebirth Wiki’s Death entry identifies Death as a Harbinger boss that can appear in Chapter 4 environments—including Womb, Utero, and Scarred Womb—after Mom has been defeated. The listed health is 450; this is wiki game data, not an independently confirmed official figure.
How Death attacks
The wiki describes an attack in which four scythes appear at the room’s walls, aligned with Isaac in the cardinal directions, then accelerate toward him. Two more scythes appear at random positions and home in on Isaac until they are destroyed.
These are the attacks documented in the available reference; they should not be treated as a complete move list or a full phase-by-phase guide. In the room, watch for the wall-aligned scythes’ approach and deal with the homing scythes when you can.

What Death drops
The Rebirth Wiki lists Ball of Bandages and Cube of Meat as Death’s drops. It also lists Locust of Death as a Repentance+ drop with a 1% chance; that chance is specific to the named edition and should not be generalized to other versions.
Can you make Death spawn?
Book of Revelations can influence which boss appears, but it does not simply guarantee Death. Using the activated item grants one Soul Heart. Depending on the floor and the game’s unlock and encounter conditions, it can cause an eligible Harbinger to replace that floor’s boss. The Harbinger selected depends on the rules for the relevant expansion and whether that Harbinger or boss has already appeared or been fought. Death the boss is not the same as dying or the Death card
If you mean what happens when Isaac dies, that is a separate game mechanic: the reference defines player death as occurring when health runs out, with some unusual exceptions. The boss entry is not a guide to those exceptions.
The XIII – Death tarot card is also a separate consumable, not the Harbinger boss. The available reference establishes that distinction but does not establish the card’s effect, so no effect is stated here.
