Combine it with a green herb to restore health; add a blue herb to that mixture to cure poison as well.
What does a red herb do by itself?
Nothing useful: it does not heal or cure a status effect on its own. Its purpose is to strengthen a herb mixture, rather than serve as a standalone healing item. The Resident Evil: The Official Strategy Guide says the red herb “doesn’t do anything by itself”; the Resident Evil HD Remaster item guide likewise describes it as useless on its own.

What happens when you mix herbs?

| Red herb alone | No useful effect by itself. |
| Green herb + red herb | Restores health. The original official strategy guide compares its healing power to a First-Aid Spray; the HD Remaster item guide says it fully restores health. |
| Green herb + red herb + blue herb | Fully restores health and cures poison, according to the HD Remaster item guide. |
The two guides describe the healing result differently in detail: the original guide says green plus red has as much healing power as a First-Aid Spray, while the HD Remaster guide explicitly says it fully restores health. Treat the full-restoration wording as specific to the HD Remaster guide rather than silently applying it to every version.
Which Resident Evil 1 version does this apply to?
The combination details here draw on the original game’s official strategy guide and an item guide for Resident Evil HD Remaster. Capcom lists HD Remaster manuals for PC, PlayStation 3, PlayStation 4, Xbox 360, and Xbox One on its official manuals page. Herb recipes can differ in later Resident Evil games, so these effects should not be assumed to apply across the series.
