The Nephalem did not vanish from Sanctuary: they are the ancestors of its human population. In Diablo IV, set 50 years after Diablo III, the broader history of humanity’s Nephalem heritage is part of the world’s past—but Blizzard has not confirmed what became of the specific hero players controlled in Diablo III.
Who are the Nephalem?
The first Nephalem were children of the renegade angels and demons who created Sanctuary. Blizzard describes them as beings with the potential to surpass their angelic and demonic parents. Humanity descended from them; “Nephalem” is therefore not simply the name of one Diablo III character, but a term for Sanctuary’s powerful early descendants and the heritage shared by its human population.

How did the Nephalem become humans?
After Lilith was banished, Inarius altered the Worldstone to suppress the Nephalem’s power. The effect unfolded across generations: descendants grew weaker and shorter-lived, and their awareness of their origins faded. In time, they became the mortal human population of Sanctuary. Blizzard recounts this history in its Nephalem lore overview and Diablo II story recap.
Did Nephalem powers return?
They were suppressed, not erased forever. During the Sin War, Lilith weakened the Worldstone’s restrictions, and Uldyssian led a movement of people whose Nephalem powers were reawakening. Uldyssian amplified that power further, then sacrificed himself to suppress it again. His fellow Nephalem lost the abilities they had regained.

Later, Baal corrupted the Worldstone, which was destroyed; Blizzard’s Diablo Immortal story recap describes that event as awakening humanity’s Nephalem heritage. This is franchise history, not evidence about the Diablo III hero’s eventual fate.
What happened to the Diablo III hero by Diablo IV?
Blizzard places Diablo IV 50 years after Diablo III in its Diablo IV story overview. A Diablo III anniversary retrospective says the Nephalem heroes continued protecting people in need after defeating Diablo, but it does not establish what happened to the playable hero over the following decades.

So the careful answer is that the hero’s later fate is unconfirmed in these official sources. They do not establish that the hero died, retired, disappeared, or appears in Diablo IV. The game’s Wanderer should not be identified as the Diablo III hero without a source: Blizzard’s overview presents the Wanderer in Diablo IV’s story but does not make that connection. Its Diablo III anniversary article supports the heroes’ activity after their victory, not a specific outcome in the era of Diablo IV.
