In the unmodded Skyrim Companions questline, refusing to drink Aela’s blood leaves The Silver Hand stuck at its blood-ritual objective. You avoid becoming a werewolf, but the quest does not proceed to the Gallows Rock mission. The alternative, if you want to continue the storyline, is to accept the transformation; a later quest offers a way to remove the curse.
What happens if you refuse Aela’s blood?
The Silver Hand requires participation in a blood ritual at the Underforge. The quest’s listed objectives proceed from the ritual to speaking with Aela and killing the werewolf hunters. The sources describe no vanilla refusal dialogue or alternate route that lets you skip the ritual and continue the quest.
If you leave or decline before drinking, the ritual objective remains incomplete and the Companions questline stalls there. You do not transform, and you do not advance to the mission at Gallows Rock.
What happens if you accept?
Drinking from the basin completes the rite and immediately transforms you into a werewolf. The next part of the quest sends you toward Gallows Rock, where Aela accompanies you against the Silver Hand. GameBanshee’s walkthrough also describes a later quest that can remove the werewolf curse.

Choosing between the two paths

If avoiding lycanthropy matters more than finishing the Companions storyline, refuse and expect to leave that quest unfinished. If you want to continue the storyline, accepting is the documented vanilla route; the later cure is separate and does not let you bypass the ritual.
