Spoilers for Metro Exodus. What happens to Artyom after Metro Exodus depends on which ending you reach: he survives and becomes the Spartan Order’s new leader in one, but dies from radiation poisoning in the other. The game does not present these outcomes as one continuous timeline.
What are Artyom’s two possible fates?
Artyom is critically ill from radiation exposure in both endings. The difference is whether the crew can secure enough blood donors to help him.

What determines which ending you get?
Player performance and choices affect the outcome. The available evidence supports that broad distinction, but not a complete list of hidden moral-point thresholds. That means no specific route or isolated choice can be promised to guarantee Artyom’s survival here.

Does Metro 2039 confirm whether Artyom survives?
No. Deep Silver’s Metro 2039 announcement names The Stranger as the game’s protagonist. Its franchise recap describes Exodus as Artyom leading Spartan Rangers away from Moscow in search of a new life in the East, but does not establish which of Exodus’s alternative endings applies afterward. The announcement therefore does not confirm that Artyom is universally alive or dead in a sequel timeline.
Is Metro: Awakening a continuation of Exodus?
No. PlayStation describes Metro: Awakening as a standalone story set before Metro 2033, so it does not continue Artyom’s post-Exodus story. A community compilation of an AMA attributes a remark to author Dmitry Glukhovsky that Artyom’s fate was already known, but the question concerned Awakening’s prequel structure; it does not settle Artyom’s fate after Exodus.

To see both outcomes for yourself, replay Metro Exodus and make different choices and perform differently across the game; the broad outcome is affected by player performance and choices, rather than one verified threshold described here. The official Metro Exodus product information documents editions, while the ninth-generation release announcement identifies Complete Edition platform details. Check the edition and platform before buying, since the linked information does not establish current stock or regional availability.
