With Secret Note #10 read, reaching floor 100 of Skull Cavern triggers a cutscene with Mr. Qi. He gives you Iridium Snake Milk, which permanently increases maximum health by 25 points. Staircase use changes Qi’s reaction, not the reward.
What happens in the Secret Note #10 encounter?
Secret Note #10 adds the “Cryptic Note” quest, which directs you to reach floor 100. Its clue reads: “Someone is waiting for you on level 100 in the skull cavern…” If you have read the note, the floor-100 encounter is a cutscene with Mr. Qi and the Iridium Snake Milk reward.

The health increase is permanent: Iridium Snake Milk adds 25 points to your maximum health.
Do staircases change the reward?
No. The number of staircases affects Mr. Qi’s dialogue, but the Iridium Snake Milk reward remains the same.

Why might Mr. Qi not appear?
The Secret Note #10 encounter depends on having read the note. Reaching floor 100 by itself does not guarantee that the cutscene will play. If you descended before finding or reading the note, the Cryptic Note quest and its encounter have not been triggered. In version 1.6, the note is no longer gated behind unlocking Qi’s Challenge.
The ordinary floor-100 treasure chest is a separate outcome. The Skull Cavern reference records a guaranteed chest on that floor from version 1.5.5 onward, except when the secret cutscene is viewed; during the cutscene encounter, the chest does not appear.

Qi’s Walnut Room has separate Special Orders that also send you to floor 100. They are not the Secret Note #10 event and have different conditions and rewards.
