It can reappear across all three acts. In Act Three, you can help it reach the Lower City and make it an ally for the final battle, or kill it for the Hat of Fire Acuity and other loot. Those outcomes are mutually exclusive: killing it means you cannot summon it as an ally.
Where to find the Strange Ox
The Ox can appear in the Emerald Grove in Act One, at Last Light Inn in Act Two, and in Rivington in Act Three if it survives earlier encounters.

Act One: Emerald Grove
Look among the other oxen in the Emerald Grove. You can question it about its secret; successful dialogue checks let it say it is traveling to Baldur’s Gate, though it does not explain why. Attacking it can turn nearby oxen hostile.
Act Two: Last Light Inn
If it survived Act One, look near Dammon at Last Light Inn. Questioning it further about its nature can make it reveal its Phasm form and turn hostile. From Act Two onward, it carries the Hat of Fire Acuity.

Act Three: Rivington
If it survived and was not attacked at Last Light Inn, find it in a stable near the Requisitioned Barn in Rivington. Speak with it before entering the Lower City: if you enter the city first, it may disappear and you may lose the chance to start its quest. Use Speak with Animals to converse with it.
Should you help the Ox or fight it?
The decision trades a final-battle ally for a powerful item. Helping keeps the Ox alive and lets you recruit it; killing it earns its Act Three loot but rules out its ally contribution.

How to recruit the Strange Ox as an ally
- In Act Three, find the Ox in the Rivington stable near the Requisitioned Barn before entering the Lower City.
- Use Speak with Animals and choose to help it reach the city.
- Pick up the Strange Apple it leaves on the stable floor; it can be easy to overlook.
- Carry the apple into the Lower City. This completes Help the Devilish Ox and makes the Ox available for Gather Your Allies.
After reaching the city, the Ox says, “Oh-ho! Thank you for the lift, my friend. Thank you very much indeed. Ta-ta!”

What the Hat of Fire Acuity does
The Hat of Fire Acuity grants Arcane Acuity for two turns whenever its wearer deals fire damage. Each remaining turn of Arcane Acuity adds +1 to spell attack rolls and spell Difficulty Class. It can be useful for a character who regularly deals fire damage, but taking it from the Ox means giving up that creature as a final-battle ally.

The Ox does not carry the hat in Act One. Killing it there yields the ring, not the Hat of Fire Acuity; the hat is carried in Acts Two and Three. Whether the hat is worth that trade depends on your party’s needs rather than a universal ranking.

Its natural form is a Phasm, an ooze-like shapeshifter, not an ordinary animal. Its ox disguise lets it recur in locations across the acts, while in Act Three it can take other combat forms when summoned.
