Standard Edition: just the base game

Premium bundles the base game with Revelations, the campaign expansion, plus a digital artbook and soundtrack and the Divinity Skin Pack. This is the straightforward option if you want the game and the listed add-ons together.
Premium Upgrade: add-ons for owners of the game
The Upgrade includes the same Premium extras—Revelations, the digital artbook and soundtrack, and the Divinity Skin Pack—but excludes the base game. Buy it only if you already own or will obtain the base game separately.

Updated DOOM Anthology: The Dark Ages with earlier games
The updated Anthology includes The Dark Ages Premium Edition, which means it includes the base game and the Revelations DLC, artbook and soundtrack, and Divinity Skin Pack. It also bundles multiple earlier DOOM titles. Bethesda says the new game is not automatically added to copies of Anthology bought before the update; earlier Xbox One and PS4 purchases cannot be upgraded to the new console bundle.

Does Revelations come with every version?
No. It is included with Premium Edition, Premium Upgrade, Collector’s Bundle, and the updated DOOM Anthology. Standard is the base game only. Revelations can also be purchased separately, but the base game is required. Bethesda’s current purchase page calls Revelations a campaign expansion; an older support page uses pre-release wording about the DLC, so the current buy-page listing is the relevant edition breakdown.
Physical copies, discs, and platform compatibility
Bethesda says PlayStation and Xbox physical copies include a disc for the base game. A physical Steam copy instead provides a code to redeem on Steam. The supported platforms are Xbox Series X|S, Xbox App for Windows PC, Steam, Battle.net, and PlayStation 5; the original release date was May 15, 2025.
