Later in the story, a Moon Stone is also given as a reward at the Joyful Game Corner on Three Island, and one sourced listing places a stone on Five Island. The sections below cover each location in the order you are likely to reach it, with notes on how each stone is picked up.
Mt. Moon: two stones on the early route
Mt. Moon is the first place most players can collect Moon Stones. Game8’s FireRed location guide says the stones here become accessible after you defeat the Pewter City Gym leader, Brock. The two stones use different pickup methods, so check both floors.

Mt. Moon 1F: visible stone in the northwest corner
Head to the northwestern corner of the first floor. The stone sits there as a visible pickup, which makes it the easier of the two to find.
Mt. Moon B2F: hidden stone on a rock
The second stone is not an item ball. On the basement level, walk along the ledge toward the Route 4 exit and stop at the eastern part of it. Search the rock itself. Poké Trainer Vault’s Mt. Moon item table independently lists this location, so it is not a single-guide claim. If you walk past the rock without interacting with it, you will miss the stone.

Celadon Rocket Hideout: a stone inside the spinning-tile maze
The Celadon Rocket Hideout is entered from beneath the Game Corner. Its B2F level contains a spinning tile maze, and the Moon Stone lies on the ground in the northwest area once you have worked through it. Game8 describes it as a visible ground pickup, so you do not need to search a rock here. The difficulty is the maze, not finding the item.
Pokémon Mansion, Cinnabar Island: a hidden stone among the rocks
Pokémon Mansion on Cinnabar Island holds a hidden stone. From the entrance, follow the carpet north until you are just before the fourth set of pillars. Turn west to reach a group of rocks, then search the northern rock. As with the Mt. Moon B2F stone, the item is hidden, so interacting with the correct rock is the step that matters.

Later-game sources: Joyful Game Corner and Five Island
Two later sources are different in kind from the map pickups above, and they should be treated separately.
Joyful Game Corner, Three Island: a quest reward
After the Lostelle sequence, an NPC at the Joyful Game Corner gives the player a Moon Stone as thanks. This is a story reward, not an item you find on the floor. Bulbapedia’s walkthrough excerpt records it. The NPC’s exact dialogue was not established from the available material, so this article does not quote it.

Five Island: a listed stone with no route
Smogon’s FireRed and LeafGreen location compendium records a Moon Stone northwest of Team Rocket’s HQ on Five Island. The entry does not give tile positions or a route to reach the spot, and no other source in the material gives those details. Treat this as a listed location to check in your own game, not a fully mapped pickup.

Using Moon Stones
Moon Stones evolve four Pokémon in FireRed and LeafGreen: Nidorina into Nidoqueen, Nidorino into Nidoking, Clefairy into Clefable, and Jigglypuff into Wigglytuff. To use one, open the Bag, select the Items pocket, choose the Moon Stone, and select Use, then pick the Pokémon to evolve. This is the process Game8 describes.
How far the sources agree
Game8 gives directions for four physical pickups: two in Mt. Moon, one in the Celadon Rocket Hideout, and one in the Pokémon Mansion. Smogon’s compendium adds the Five Island stone, and the Bulbapedia walkthrough excerpt adds the Joyful Game Corner reward. Because these sources cover different scope, the three areas in Game8’s heading do not account for every possible Moon Stone. If you are checking a full list, count the Joyful Game Corner reward and the Five Island entry separately from the floor pickups.
The exact Five Island route and the Joyful Game Corner dialogue are the two open points. Everything else in this guide is supported by the sources named above.
A printed FireRed and LeafGreen walkthrough from 2004 also exists, but none of the location details above depend on it.
Clear the Mt. Moon stones first, then work through Celadon and Cinnabar in any order, and leave the Joyful Game Corner reward and the Five Island entry for later in the story.
