Across the base campaign and both story expansions, 15 named characters die or suffer a death-like fate. The list includes heroes, villains, spirits and one character whose body is reused after his sacrifice. Mephisto is not a confirmed death: he is banished to the Void between realms and described as neither dead nor alive.
What counts as a death in the Age of Hatred story?
This roster follows the three-part structure used by a June 25, 2026 Windows Central retrospective: the Diablo IV base campaign, Vessel of Hatred and Lord of Hatred. It separates an explicit death from a mortal wound, an undying remnant and a spirit sacrifice, because those distinctions change what happens to the character afterward.

Diablo IV base-campaign deaths
Vhenard
Vhenard, Neyrelle’s mother, is corrupted by Lilith’s promise of forbidden demonic knowledge. Her transformation makes her a direct threat rather than a simple victim. After Vhenard dies, Neyrelle briefly raises her so that her mother can help open the way forward. That temporary return does not undo the death; it is a supernatural intervention tied to the campaign’s progression.

Vigo is the watch commander who chooses to hold back corruption so the others can pass. He wears the penitent armor known as Father’s Grace and gives his life to cut a path through the infestation. His fate is an intentional sacrifice, not an off-screen disappearance.
#1 Best Overall
Rathma
Rathma, the son of Inarius, refuses to surrender the Hell Key. Inarius responds by killing his own child. The scene establishes the personal fracture at the center of the Cathedral of Light and makes Rathma’s death a direct consequence of the conflict between father, son and the forces gathering around Sanctuary.
Yorin
Lilith uses a soulstone to let Astaroth possess Yorin. The possession turns Yorin into a vessel for the demon’s return. Once Astaroth is defeated, Yorin is already dead; defeating the demon does not restore him. His fate therefore combines a magical takeover with a permanent human loss.
Meshif
Meshif, the adventurer familiar from earlier Diablo stories, returns during the Kehjistan portion of the campaign. He is cut down while helping the Wanderer escape a palace. His death occurs during the escape itself, making his final act one of assistance rather than a separate boss encounter.
Elias
Elias is defeated in the Black Tomb of Sankekur, but his fate does not follow an ordinary corpse-and-funeral pattern. The Tree of Whispers takes his head, which remains undying. It is more accurate to call Elias defeated and reduced to an undying head than to describe him as conventionally dead. The remnant matters because the Tree becomes the place where his punishment and continued existence are contained.

Donan
Donan helps the group entrap Mephisto in the soulstone in Hell. During that effort he sustains a mortal wound. The story describes the wound as mortal, so “fatally wounded” is the precise formulation: the account does not present a conventional death scene in the same terms used for Inarius or Lilith. Donan’s injury is nevertheless treated as the end of his survival.
Inarius
Inarius confronts Lilith in Hell and loses. She impales him with his own spear and tears off his wings. The image reverses the power relationship between the creator of the Cathedral of Light and the daughter of hatred, while making the cause of death unmistakable: Lilith kills him directly.
Lilith
The Wanderer defeats Lilith in the Cathedral of Hatred. After the fight, she turns to dust. Before disappearing, Lilith warns that without her there would be no victory. Her destruction ends the immediate campaign conflict, but the warning also frames the consequences that follow: removing Lilith does not remove every threat tied to Mephisto and the soulstone.
Vessel of Hatred deaths
Vessel of Hatred begins with Neyrelle carrying Mephisto’s soulstone south to Nahantu. Blizzard’s setup places the player’s search for her at the center of the expansion: the Wanderer must find Neyrelle and join her against Mephisto’s plans. The following deaths occur as that burden and the Nahantu conflict escalate.

Urivar
Urivar, the Knight Penitent, is killed by the Wanderer in their final confrontation. Unlike Elias or Akarat, Urivar has no surviving spirit, body-reuse twist or unresolved state attached to the description. His arc ends with a direct defeat and death at the player character’s hands.
Maka
Maka is a member of the Spiritborn council who tries to help Neyrelle resist Mephisto. The evil within Neyrelle lashes out during that attempt and kills Maka. Her death shows that Neyrelle’s possession-like burden endangers allies as well as the person carrying the soulstone; Maka is not killed by a separate battlefield opponent.
Akarat
Akarat’s fate has two distinct parts. First, his spirit sacrifices itself to defeat Mephisto’s harbinger. Later, Mephisto steals Akarat’s body and uses it to walk Sanctuary. Calling Akarat simply “dead” loses the important distinction: the spirit is spent in the sacrifice, while the physical body becomes an instrument for Mephisto.
Lord of Hatred deaths and unresolved fates
Neyrelle
Blizzard’s Neyrelle tribute confirms that she dies and honors the choice that led her to carry Mephisto’s soulstone. That tribute expressly does not reveal how she died. A later Windows Central retrospective supplies a further account: Neyrelle finds a way to stop Mephisto, sends word to her friends, and is then killed by Mephisto. These statements should be kept separate: Blizzard confirms the death without specifying its mechanics, while the retrospective attributes the Mephisto killing to the later story account.

Lorath’s writings, reproduced in Blizzard’s tribute, address the person Neyrelle remains despite the burden: “There can be no doubt of your strength and your goodness. You are still Neyrelle. You are something more than Horadrim.” The tribute presents those words as Lorath’s writing; it does not supply a separate publication date for the in-world document.
Queen Adreona
Mephisto misleads Queen Adreona while wearing Akarat’s body. Under that deception, Adreona kills Lorath. She is later struck down by Lorath’s allies on Mount Hefaetrus. Her death is therefore part of a linked sequence: Mephisto’s manipulation causes the first killing, and the survivors’ retaliation ends Adreona’s life.
Lorath Nahr
Adreona kills Lorath. His head is taken to the Tree of Whispers, echoing the story’s use of the Tree as a repository for the remains of dangerous or important figures. Lorath’s allies later set fire to the Tree and lay him to rest. The funeral action gives his death a final memorial resolution even though his body is initially treated in the same unsettling way as Elias’s head.

Mephisto
Mephisto is not one of the saga’s confirmed dead characters. He is stabbed with a weapon made from Lilith’s body and banished into the Void between realms. The retrospective characterizes his state as neither dead nor alive. He belongs in a complete fate list because the confrontation removes him from Sanctuary for the moment, but counting him as a death would overstate what the story establishes.
Best Value
The distinctions that prevent common spoiler-list mistakes
Confirmed death versus a mortal wound
Donan’s outcome is described as a mortal wound, not with the same scene-by-scene finality used for Lilith, Inarius or Urivar. A precise spoiler summary should say he is fatally wounded. That wording preserves the qualification supplied by the story instead of converting it into an unsupported conventional-death claim.
Body, spirit and demonic essence are not interchangeable
Akarat’s spirit is sacrificed while his body is later used by Mephisto. Elias retains an undying head after being defeated. Neyrelle’s death is confirmed, but the source that confirms it does not explain the circumstances. These are different narrative states, and collapsing them into a single “killed” category hides why each fate matters.
Why Mephisto should not be listed among the dead
The final confrontation changes Mephisto’s location and condition rather than establishing a straightforward death. “Banished to the Void” is the supported conclusion; “killed” is not. Future story developments could clarify his status, but the Age of Hatred account itself leaves him in the neither-dead-nor-alive category.

The Age of Hatred saga’s named losses run from Vhenard and Vigo in the early campaign to Neyrelle, Lorath and Adreona in Lord of Hatred. Rathma, Meshif, Inarius, Lilith, Urivar and Maka have direct killing circumstances; Yorin dies after Astaroth’s possession; Elias leaves an undying head; Donan suffers a mortal wound; and Akarat’s spirit and body follow separate paths. Neyrelle’s death is confirmed by Blizzard, with the later Mephisto account coming from the retrospective. Mephisto himself is banished, not confirmed dead.
