Fingers bend, overlap, disappear behind objects and appear at many angles. A small mistake in shape, pose or visibility can make the whole hand look implausible. That does not mean every generated hand is wrong, or that all image generators fail in the same way.
What goes wrong when AI generates hands?
Errors go beyond simply getting the number of fingers wrong. Researchers have documented irregular hand shapes and poses, missing or extra digits, unusual relative finger lengths, and fingers facing physically implausible directions. A hand can also have the right apparent number of digits but still look incorrect because its proportions or arrangement do not make anatomical sense.
A 2024 study evaluated 240 photorealistic images generated by DALL-E 3, Stable Diffusion XL and Stable Cascade. The images came from 10 prompts, with eight images per prompt. The authors classified errors including proportion, extra-part, orientation, configuration and missing-part problems, and showed examples such as a six-fingered hand. This is a description of that study’s sample—not a universal error rate or a measure of how often all AI-generated hands fail. Read the study in PLOS ONE.
Why are hands difficult to depict?
A hand is a complex 3D structure in a 2D image
Hands have many joints and possible poses, and an image generator has to depict them from visual patterns in a flat image. Fingers may bend, cross or hide behind one another; the palm may be turned partly away from the viewer. When an object blocks part of the hand, the image offers even less visible information about its shape. HandRefiner’s authors identify both hand structure and the challenge of translating three-dimensional visibility into a two-dimensional image as difficulties for generation. In a BBC Science Focus interview published February 4, 2023, University College London computer scientist and author Professor Peter Bentley put the distinction plainly: “These are 2D image generators that have absolutely no concept of the three-dimensional geometry of something like a hand.” He said models had learned the general idea of a hand—“a palm, fingers and nails”—but did not understand the complete object as a 3D structure.
Overlapping fingers create ambiguity
When fingers cross or overlap, the generator must produce a coherent answer to which finger is in front, which is behind, and how each connects to the palm. Occlusion can make those relationships hard to infer from the visible image. A wrong overlap can turn an otherwise plausible pose into a tangle of shapes.

Objects and gestures add more constraints
A hand holding a camera or knife must fit around the object; a gesture or interlaced pose requires several fingers to occupy specific positions relative to each other. These extra relationships make the image harder to keep coherent. Bentley described this context as a source of difficulty and recalled asking DALL-E for interlaced hands, only to get what looked like “two wrists and a ball of fingers.” That is an example from an interview, not a controlled comparison of prompts.
What are researchers doing to improve generated hands?
Research approaches include giving a generator explicit hand or body pose information, and editing an existing image to repair a malformed hand. These are distinct strategies: one aims to guide generation with structural information; the other targets a hand that is already in the image. The cited papers document research methods, not proof that any named consumer image generator currently offers them.

Conditioning generation on hand parameters
The 2024 CVPR paper HanDiffuser describes a two-part system: a text-to-hand-parameters diffusion model generates body and hand parameters, then a text-guided image synthesis model uses those parameters as conditioning. The aim is to give image synthesis more explicit structural guidance than a text description alone.
A separate approach, Hand1000, uses hand-gesture representations and fine-tuning to generate more realistic hands from text. Both illustrate research into providing specialized hand information; neither establishes that the same controls are available in mainstream products.

Repairing a hand in an existing image
HandRefiner describes conditional inpainting to refine malformed generated hands. Inpainting edits a selected area while using surrounding image content as context, which can be useful when the rest of a picture should remain intact. The paper’s discussion addresses the need to resolve hand structure and which finger appears in front when fingers cross.
The 2025 WACV paper HandCraft describes a restoration method that uses hand masks and depth images as conditioning signals for image editing. Its goal is to correct hand anatomy and pose while preserving the surrounding image.

Do prompt changes reliably fix bad hands?
The cited research supports specialized structural conditioning and image-editing methods, but it does not establish that changing a prompt reliably corrects malformed hands. A prompt may describe a pose or interaction, yet the available evidence here does not provide a fair, current comparison of consumer tools or prove that prompt adjustments solve the underlying geometry and overlap problems.
Likewise, the evidence does not rank today’s image generators or establish a universal failure percentage. The 2024 sample shows specific error types across three named systems under its study conditions; it should not be treated as a current cross-product benchmark.
Can AI-generated hands be used in medical or educational images?
Not without qualified review. A study of AI-generated hand-surgery patient-education images found limitations in anatomical realism and fabricated anatomy. That evidence concerns patient-education imagery, rather than every creative use of image generation, but it is a clear reason not to treat a generated anatomical image as medically reliable without expert assessment.
