Kasumi Nakano ran away because she feared she might be a synth who had replaced the real Kasumi. Feeling out of place in her family, uncertain about her childhood memories, and troubled by dreams of waking in a laboratory, she went to Acadia after hearing from DiMA. The story presents this as Kasumi’s suspicion—not proof that she is a synth.
What made Kasumi leave home?
Kasumi is the daughter of Kenji and Rei Nakano. She had helped her grandfather restore appliances, but increasingly felt she did not belong with her family. Gaps and uncertainty in her childhood memories, along with dreams about waking in a laboratory, fed her fear that she might be a synth. The character reference describes her as doubtful and lacking proof; the game does not establish that her fear is fact.
Why did she go to Acadia?
A radio signal brought Kasumi into contact with DiMA, who drew her toward Acadia and its synth residents. She also wanted to learn more about DiMA’s memories. Acadia offered a place where she could investigate her questions about herself, away from home.
Acadia is a synth colony and refuge in Far Harbor, not a conventional fallout shelter. Bethesda describes the add-on as a search for a young woman and a secret synth colony.

What happens after the Sole Survivor finds her?
When the player tells Kasumi’s parents they have found her, Kasumi says she will not consider returning home until the problems on the island are dealt with. Her interest in DiMA’s memories is part of why she wants to stay. The dialogue transcript records this exchange.
Her decision places her in the middle of Far Harbor’s broader tensions among Acadia’s synths, the Children of Atom, and the island’s townspeople. Bethesda says player choices affect the groups’ fates; as lead designer William Shen put it, “Everyone has a sympathetic side, and a side that’s not-so-sympathetic, and you have to decide where you’re going to land.”

Kasumi’s story is part of Fallout 4: Far Harbor. Bethesda’s 2017 announcement says Fallout 4: Game of the Year Edition includes the base game and all six add-ons, including Far Harbor. Check Bethesda’s edition announcement; confirm platform and physical or digital format with the seller.
