Find and replace yours safely.Free scan · under a minuteFallout 4 crashes are usually caused by one of five things: damaged or mismatched files, drivers or graphics settings, broken configuration files, incompatible mods or Creations, or a problem with a save, system, or permissions. The fastest safe diagnosis is a clean-baseline test: back up saves, disable all added content, remove launch options, verify the installation, regenerate configuration files, and test a new game before restoring changes.
First, identify exactly when it crashes
The timing and pattern narrow the likely cause:

The safe clean-baseline test
1. Back up saves and mod profiles
On a standard PC installation, copy C:Users<username>DocumentsMy GamesFallout4 to another location before changing INI files, removing content, or testing old saves. Also preserve your mod-manager profile, load-order export, F4SE settings, and custom INI files. Do not overwrite your only backup with a save that may depend on removed content. Bethesda documents this save location at its crash troubleshooting page.

2. Disable every mod and Creation
For the test, disable third-party plugins, texture and mesh replacements, settlement overhauls, UI mods, script-heavy quests, F4SE plugins, ENB/ReShade files, and injector DLLs. In the in-game Creations menu, disable or delete active Creations. Bethesda notes that the separately installed Creations Bundle in the Anniversary Edition is not removed merely by disabling or deleting individual Creations; see its Creations guidance.
Disabling a plugin may not remove loose files or DLLs. A clean test therefore requires the unmodded executable and files, not just one unchecked mod.

3. Remove Steam launch options
- Open Steam and select Library.
- Right-click Fallout 4 and choose Properties.
- In General, delete everything in Launch Options.
- Close the window and start the game normally.
Old commands may target a downgraded executable or obsolete modlist. Bethesda documents this procedure at its PC crash page.
4. Verify official files
- In Steam, open Library, right-click Fallout 4, and select Properties.
- Choose Installed Files.
- Select Verify integrity of game files and wait for completion.
For GOG, Epic, and the Xbox app, use that platform’s equivalent repair or verification command. Verification repairs official files but does not reconstruct a mod load order and may replace files expected by a downgrade or manual mod installation. Bethesda says one or more files failing verification can be normal because local configuration files may not be replaced.
5. Regenerate INI files
After backing up C:Users<username>DocumentsMy GamesFallout4, rename these files:

Start the launcher, let it detect hardware, accept default settings, and test a new game. Mod managers that use profile-specific INIs may load different files, so check the manager’s profile settings before assuming these are active.
6. Test a new game
- New game also crashes: investigate files, drivers, configuration, hardware, or still-active content.
- New game works but an old save crashes: suspect save corruption, a missing dependency, or a broken quest state.
- Only one menu crashes: suspect UI, HUD, inventory, or Scaleform mods.
Removing a mod from an established save can make that save worse. Use the clean test to identify the cause, then restore a known-good profile rather than repeatedly stripping content from your main playthrough. Bethesda explains missing-content warnings at its save-dependency page.
PC fixes after the baseline test
Update, clean-install, or roll back the GPU driver
Download drivers from the GPU manufacturer, not a third-party updater. Bethesda points users to NVIDIA and AMD support. A clean installation can remove corrupted profiles; if crashes began immediately after an update, a rollback may be more appropriate. Laptop owners may need the computer manufacturer’s package.

Return overclocks and undervolts to stock. Temporarily disable Steam, Discord, NVIDIA, AMD, Xbox Game Bar, MSI Afterburner, ReShade, ENB, and other overlays or injectors. Reset launcher-detected video settings and try lower resolution or quality settings as a diagnostic, not as a cure for broken plugins or saves. Bethesda’s graphics guidance is at this support page.
Repair legacy DirectX components
A current Windows installation with DirectX 12 may still lack older components used by Fallout 4. Use Microsoft’s official download search at Microsoft DirectX downloads. Never obtain individual DLL files from random websites.
Check security software and permissions
Antivirus or firewall rules can interfere with launching or saving. If necessary, add narrowly scoped exceptions for trusted files such as Fallout4.exe, Fallout4Launcher.exe, f4se_loader.exe, and the relevant game or mod-manager folder. Do not disable protection permanently. Bethesda discusses save/load interference at its save-operation page. A library outside Program Files can simplify permissions for heavily modded setups, but relocating the game is not a universal crash fix.

Modded Fallout 4: runtime and load-order diagnosis
Match F4SE to the actual runtime
F4SE is not universal. Its published notes support Steam, with separate GOG information; standard F4SE does not support Microsoft Store, Game Pass, or Epic installations. Never give F4SE instructions to console or unsupported PC editions. Check the game runtime before installing it. The files listing observed on May 27, 2026 showed:

These are listings, not a permanent universal version number. Recheck the F4SE files page before installing. Extract F4SE into the folder containing Fallout4.exe, launch with f4se_loader.exe, and enter getf4seversion in the in-game console to verify it, following the published instructions.
Audit native-code plugins
A working F4SE installation does not mean every DLL plugin is compatible. Authors must update native plugins for newer runtimes such as 1.10.980+ and 1.11+. Check every plugin’s supported runtime, remove outdated DLLs, and test with all F4SE plugins absent if the crash occurs before the menu.

Isolate ordinary mods systematically
Check missing masters and load-order warnings, then re-enable content in batches. If a batch reintroduces the crash, divide that batch in half and repeat. This identifies a conflict without repeatedly damaging a save. Keep separate profiles for the current runtime, downgraded modlists, and a no-mod baseline.
Use crash logs as clues
Tools such as Buffout 4 and Crash Log Auto Scanner are PC-only and require a compatible F4SE environment. Community guidance is available at Nexus Mods’ stability article. Logs can contain EXCEPTION_ACCESS_VIOLATION, VCRUNTIME140.dll, BSScaleformImageLoader, or Fallout4.exe; none proves the root cause. Treat a named DLL or plugin as a lead, not a verdict, and follow the tool’s exact runtime requirements.
Xbox and PlayStation troubleshooting
- Exit Fallout 4 completely rather than leaving it suspended.
- Restart the console.
- Disable all Creations or mods.
- Test a new game and compare it with the affected save.
- Confirm the game, DLC, and save use the same edition and platform version.
- Protect cloud and local saves before reinstalling.
- Reinstall only after the no-Creation test and basic restart fail.
Bethesda recommends fully exiting and rebooting for general in-game errors at its console troubleshooting page. Console players cannot use F4SE, Buffout 4, or PC crash-log tools. Deleting reserved storage can remove locally stored Creations, so do not do it without a current account or cloud backup.
When reinstalling is justified
Reinstall only after saves are backed up, verification has been attempted, INIs have been regenerated, drivers and DirectX have been checked, and all mods and Creations are removed. A reinstall may repair damaged official files, but it does not necessarily remove user configuration, manually installed loose files, mod profiles, or a broken save. Test the untouched installation before restoring any added content.

Contact Bethesda or platform support when the game still crashes in a clean, unmodded new game after platform repair steps. Include your platform and edition, runtime, crash timing, reproduction steps, whether a new game crashes, and the verification or reinstall steps completed. For PC cases, Bethesda may request a DxDiag report; its driver-crash guidance is at Bethesda Support.
The Bottom Line
Do not start with a random crash-fix mod or a reinstall. Back up saves, establish a no-mod baseline, verify files, regenerate INIs, test a new game, and then reintroduce drivers, runtime components, and mods one controlled group at a time.
