What happened to the real Simon?
Before the main events, Simon is hit by a car while trying to help an injured man. He survives, but the accident leaves him unable to walk. A detailed plot account posted on the Cry of Fear forum says psychiatrist Doctor Purnell recommends that Simon write a book as therapy; the events that follow are presented as a metaphorical journey through his distress. Why can the playable Simon walk?
The walking Simon encountered through much of the game is not simply the real Simon moving around in the physical world. The forum account describes him as a mental projection associated with the book Simon is writing. That distinction makes the late reveal fit: the game has shown a symbolic inner world, then exposes the wheelchair-bound person behind it.

Does Simon recover in the fourth ending?
No. In the fourth ending, Simon remains in his wheelchair. The forum plot summary says this ending is unlocked by killing Carcass and trusting Doctor Purnell. Simon confronts the book-version of himself during a psychosis, but the apparent victory gives way to the revelation that he has shot two police officers, mistaking them for his hallucinated opponent. He is committed to a mental hospital, where Purnell continues as his counselor and Sophie visits when permitted. The ending changes Simon’s circumstances and relationship to his book-self; it does not show him walking again. The ending details come from the forum summary.
How does the co-op ending differ?
The same forum account describes the co-op ending as preventing the accident: police reach the scene before Simon is hit. It also treats that ending’s canon status as uncertain. It is a separate outcome, not evidence that the main story’s wheelchair reveal never happened.
