What are the hardest video game achievements to unlock? This editorial ranking spans punishing no-death runs, restrictive difficulty modes, team challenges, marathon grinds and even a five-year wait. The order follows FandomWire’s November 6, 2025 list; it is not an objective score, and “hardest” depends on whether you weigh execution, time, coordination or randomness most heavily.

Each entry below is ranked in the order used by FandomWire. Requirements can vary by platform, edition or achievement record, so check the version you play before planning a run.

LASO Master — Halo: The Master Chief Collection
Complete every campaign in the collection on Legendary with every Skull active. The modifiers make enemies more dangerous and limit tools such as radar and shields. FandomWire says checkpoints are unavailable unless playing with a squad, so this challenge combines a harsh ruleset with a long series of campaign runs. FandomWire’s ranking describes the requirement.

The Legendary Hero — Metal Gear Solid 4: Guns of the Patriots
This is a demanding set of restrictions on the hardest setting, with a completion time under five hours. The exact list of conditions is not consistent across the two editorial accounts: FandomWire describes eight playthroughs and says not to be detected, heal or die; GameSpot describes a hardest-difficulty run with no kills, health packs or detection, after earning the other trophies. Treat the details as edition- and record-dependent rather than assuming these accounts describe identical requirements. GameSpot’s account covers the alternative description.

Finish the game on AKUMU difficulty, where a single hit can kill and resources are scarce, according to FandomWire. Its November 2025 article reported a 2.2% Steam completion rate, citing Steam; that is a dated, platform-specific figure, not a current rate or a universal measure of difficulty. FandomWire’s entry includes the reported percentage.

Go Outside — The Stanley Parable
Wait five years without playing the game. Unlike a precision challenge, this achievement tests patience and requires a long real-world time gate.

Masterminds — Grand Theft Auto Online
Complete three Criminal Mastermind scenarios on the hardest difficulty with the same team, without losing a life during setups or finales. The challenge depends on reliable coordination as well as individual skill: one mistake can undermine a group’s progress. GameSpot also emphasizes the no-lost-lives requirement and need for a dependable team. GameSpot’s overview discusses the challenge.

My Kung Fu Is Stronger — Mortal Kombat (2011)
This achievement is a large cumulative grind across the game’s characters. GameSpot specifies 24 hours with each character, 100 wins, 100 fatalities, 150 X-Ray hits and 10,000 liters of blood. FandomWire likewise describes extensive character-specific play and actions. Check that the achievement and requirements match your version of the 2011 game before tracking progress. Complete the game on Authentic Plus without manual saves. FandomWire describes a mode with the HUD and radar disabled and scarce supplies. It reported a 1.1% Steam completion rate in November 2025; that figure is a dated report, not a live statistic. FandomWire’s entry gives the reported rate.

No Point in Dying — Limbo
Finish the game in one session with no more than five deaths. A single-session requirement turns a short puzzle-platformer into a careful endurance run: mistakes count against a strict limit. GameSpot estimates a three-to-four-hour run. GameSpot’s description also specifies the session and death limit.

Impossible Boy — Super Meat Boy
Complete a Cotton Alley Dark World level without dying. FandomWire says a death sends you back to the start of that level, making this a focused test of consistent platforming rather than a long campaign-wide no-death run.

The Shadows Rushed Me — Max Payne 3
Finish New York Minute on Hardcore in one sitting, without dying, while beating the in-game timer. GameSpot characterizes it as a no-save, no-death campaign with a strict time limit. The combination leaves little room to recover from either a slow section or a fatal mistake. GameSpot’s account describes the time pressure.

Those descriptions do not establish one identical, more precise rule set, so verify the achievement requirements for your platform before attempting it. GameSpot’s description offers its version of the conditions.

Mein Leben — Wolfenstein II: The New Colossus
Complete the game on Mein Leben difficulty without dying. FandomWire says a death restarts the run from the beginning, making every encounter part of one high-stakes attempt rather than a sequence of recoverable checkpoints.

The Bladder of Steel Award — Rock Band 2
Complete the 84-song Endless Setlist 2 without pausing or failing a song. FandomWire estimates six to seven hours, while GameSpot describes a run lasting more than six hours. This is as much a sustained concentration and endurance test as a musical one. GameSpot’s entry also gives the 84-song requirement.

Seriously 5.0 — Gears 5
FandomWire lists several distinct demands: finish the campaign on Insane, complete Master Horde and Escape Launch maps, reach 20 Re-Ups and earn General rank in Tour of Duty. GameSpot describes the same broad mix of campaign skill, high-level modes and long-term progression. It is a breadth-and-commitment challenge, not a single difficult final encounter. What makes an achievement “hard”?
There is no shared scoring method behind this ranking. These achievements test different kinds of difficulty, so a player who is comfortable with high-pressure combat may find a long grind or a team-dependent challenge worse.
- Execution and failure penalty: Mein Leben, Impossible Boy, The Shadows Rushed Me and You Asked For It punish errors through strict death limits or demanding runs.
- Difficulty modifiers: LASO, AKUMU, Authentic Plus and Mein Leben impose unusually restrictive or punishing conditions.
- Time and repetition: My Kung Fu Is Stronger and Seriously 5.0 require extensive cumulative progress; Go Outside is an outright real-world time gate.
- Coordination and endurance: Masterminds relies on a consistent team, while The Bladder of Steel Award demands a continuous 84-song effort.
- Randomness and breadth: Master Zoologist combines a large collection target with the challenge of locating creatures.
How to use this ranking
FandomWire’s list is an editorial selection, not a measured comparison of every achievement across every game. GameSpot’s overlapping coverage supports several descriptions but also shows why exact requirements should be checked against the achievement record for the platform and edition you intend to play. For Xbox, Microsoft’s certification policy says achievements must be earned through in-game actions, gameplay or experiences, and that achievements defined by the base title must be earnable without additional in-title purchases. Those are Xbox rules, not universal requirements for Steam, PlayStation or every game. Microsoft’s Xbox certification requirements explain the platform policy.
