If you want a game with P.T.’s claustrophobic atmosphere, gradually tightening dread, first-person exploration, or environmental puzzles, start with Visage. Its developer explicitly says P.T. was a major influence. MADiSON is a strong fit for camera-led exploration and puzzles; Layers of Fear for a domestic space that seems to warp around you; and SOMA for investigative, science-fiction horror. None is an objective replacement for P.T.—each emphasizes a different part of the experience.

These are different kinds of horror, not entries on a single “scariest” scale. Choose by the element you want most: an uncanny house, puzzle-focused exploration, shifting architecture, or a science-fiction mystery.
Visage: the closest documented connection to P.T.
SadSquare Studio, the developer of Visage, directly identifies P.T. as a major inspiration. The studio says: “Though we were heavily inspired by the brilliant and matchless P.T., we also drew upon influences from classic psychological horror games like Phantasmagoria and from more recent ones like Amnesia: The Dark Descent, as well as from horror movies like The Grudge.” SadSquare Studio’s site makes this the clearest recommendation if you specifically want to follow P.T.’s influence.
Visage takes place in a house with a terrible history. Xbox’s description emphasizes slow tension, exploration, environmental clues, a changing house, and a sanity mechanic. The player is defenseless and must avoid or evade threats, so the appeal is not combat mastery but enduring an increasingly unsettling place.

MADiSON: camera puzzles and beyond-facing exploration
MADiSON is a first-person psychological horror game whose central tool is a possessed instant camera. PlayStation describes using it to connect the human world with the beyond, solve puzzles, explore, and survive. That makes it the most direct pick here if you want the act of investigating a space and manipulating clues to be central to play.
The official MADiSON site calls it a “FIRST PERSON PSYCHOLOGICAL HORROR GAME” and says “OUT NOW” (page observed October 7, 2026). If what stayed with you in P.T. was the sense that a familiar domestic space could become impossible to trust, Layers of Fear is worth considering.
