There is no verified, official, up-to-date master list of every quest item in Path of Exile 2. Community indexes and guides are useful starting points, but their counts and item details can change with patches. The key distinction is what an item asks you to do: advance an objective, hand it to someone, use it at a site, or consume it for a lasting character bonus.
Is there a complete list of PoE 2 quest items?
Not one established as both official and complete. The Path of Exile 2 Wiki quest-item index is community maintained. Game8’s guide describes itself as a list of 37 quest items, but that is the guide’s count, not an official total from Grinding Gear Games (GGG). Neither source should be treated as proof that its list is exhaustive for the current game version.

GGG’s December 10, 2025 item-filter announcement documents item classes, additions, renames, and removals for that update. It helps show why names and categories can shift, but it is not a complete walkthrough of quest objects, their sources, or their uses. For any specific item, use the in-game quest journal and current item description to confirm what to do.
Quest objects do different jobs
The label “quest item” does not by itself tell you whether an object is mandatory or what happens when you interact with it. Read the objective alongside the item’s description and distinguish these uses:

- Progression object: collect it or bring it to an NPC to advance an objective.
- Site or device offering: carry it to the named altar, ritual site, or other destination and use it there.
- Reward object: use or consume it to gain an effect that remains on your character.
Do not assume that every indexed quest item unlocks an area or must be turned in. Conversely, an item that disappears after use may have granted a lasting bonus rather than being only a temporary quest key.
Spirit quest items: the documented campaign example
In a December 22, 2024 official forum post, GGG staff member Wolf_GGG explained the intended Spirit-item rewards for the campaign setup described at that time:
“We intentionally expect Spirit quest items dropped in the campaign to work as follows:
– Mist King Act 1 Normal -> 30 Spirit item
– Ignagduk Act 3 Normal -> 30 Spirit item
– Mist King Act 1 Cruel -> No Spirit item
– Ignagduk Act 3 Cruel -> 40 Spirit item
The base total of Spirit you should have for clearing the above bosses should be 100.”
This is a version-specific explanation, not a timeless guarantee for every later patch. In the described setup, killing the King in the Mists again in Cruel does not provide another Spirit item, while Ignagduk in Cruel provides the 40-Spirit item. The 100 base Spirit total refers only to clearing those listed boss encounters. See Wolf_GGG’s original forum post for the dated explanation.

The community index and Game8 guide name Gembloom Skull, Gemrot Skull, and Sacrificial Heart. Game8 describes the two skulls as Spirit-related quest items, and says the Sacrificial Heart is used at the proper ritual site for two Weapon Set Passive Skill Points. These are secondary-guide details and may be patch-sensitive; confirm the current drop source and use location in the game before relying on them.

How to check what to do with an item
- Open the quest journal and find the active objective that mentions the item or its destination.
- Read the item description and identify whether it names an NPC, altar, ritual site, device, or reward effect.
- Go to the stated destination and interact with the relevant NPC or object; do not assume a pickup automatically completes the objective.
- After using it, check the journal and character bonuses to see whether the quest advanced or a lasting effect was granted.
If a guide’s instructions do not match your current objective, follow the in-game journal for your version and difficulty. Item names, categories, and quest behavior can change across updates.
