After completing The Cold Flame of Agnon, you can pursue one of two Shivering Isles succession rituals: Mania or Dementia. The choice determines which ruler you replace—you cannot become ruler of both Houses through the same succession decision. To rule Mania, poison Thadon with Greenmote and offer his blood at the Sacellum; to rule Dementia, kill Syl and offer her heart.
When can you become a Duke or Duchess?
Succession becomes available after The Cold Flame of Agnon. Speak with Sheogorath and choose either the Ritual of Mania or the Ritual of Dementia. Each route has its own target and ritual item, and completing your chosen route advances the main quest to Retaking the Fringe. The surviving ruler reacts to the other ruler’s death.
How to become Duke or Duchess of Mania
The Mania ritual requires Thadon’s blood. To obtain it, you must steal Greenmote and use it to poison both his meal and wine. The quest steps below follow GameSpot’s 2007 Shivering Isles walkthrough, with quest details cross-checked against Elder Scrolls Wiki’s quest reference.

After The Cold Flame of Agnon, speak with Sheogorath and choose the Mania succession path. Then speak with Dervenin, High Priest of Mania, to learn the ritual: you must bring Thadon’s tainted blood to the Sacellum altar.

Speak with Wide-Eye and follow her into the Greenmote Silo. The walkthrough places her routine around noon. Take Greenmote from the silo and leave without being caught.

Go to Thadon’s quarters in the House of Mania. Poison both his meal and his wine with the Greenmote.

Return to the feast around 8 p.m. and wait for Thadon to die. Search his body and take his blood.

Bring the blood to the Sacellum and use it at the Altar of Arden-Sul. The Mania priest proclaims you Duke or Duchess of Mania. Speak with Sheogorath afterward to receive the Ring of Lordship and continue the questline.

Getting through the guarded areas
The Greenmote Silo and Thadon’s quarters are guarded. Stealth or invisibility can help you avoid detection, but neither is a stated requirement of the ritual.
How to become Duke or Duchess of Dementia
The Dementia route replaces Thadon’s poisoning ritual with a confrontation involving Syl. Speak with the Dementia priest and the relevant House of Dementia advisers, then pursue Syl through her escape route. Kill her, take her heart, and return it to the Sacellum altar to complete the ritual and claim the Dementia title. This is the alternative succession outcome, not an additional title earned after the Mania route.

Choosing between the two rituals

Both choices lead onward to Retaking the Fringe. Pick the House you want to rule; the succession choice does not award both titles.

If you have not entered the expansion yet, Bethesda Support’s instructions for the original expansion say to install Shivering Isles, wait 24 or 48 in-game hours, travel to Bravil, and swim to the island east of the city. Approach the doorway after the guard encounter and the NPC’s death; the doorway’s invitation speech marks the point at which you can enter.
