No: Toad is not wearing a hat. In a Nintendo of America interview published on February 5, 2018, Super Mario Odyssey producer Yoshiaki Koizumi said the mushroom-shaped top is Toad’s head.
What Yoshiaki Koizumi said about Toad
The question put to Koizumi was whether Toad was wearing a hat or whether the mushroom-shaped top was his head. His answer was direct: “So that, as it turns out, is actually Toad’s head.” Crunchyroll News reported the answer, and Destructoid reproduced the fan question and response.
What the answer does—and doesn’t—explain
Koizumi did not give a detailed biological explanation for how Toad’s head works. He left that for viewers to figure out, adding, “Maybe there’s something inside…” That remark is speculation, not a further confirmed account of Toad’s anatomy.
When the clarification was made
This was a fan-question answer in a Nintendo of America interview video, not a new announcement. Contemporaneous coverage dates Nintendo of America’s post to February 5, 2018, and identifies Koizumi as the producer of Super Mario Odyssey. Nintendo Life also reported the Toad clarification.
