The crossover changes the look of your cards, not the game’s poker-based mechanics, and the official announcement said the update was available on Steam, consoles and mobile.
What the Binding of Isaac crossover adds to Balatro
The Isaac-inspired artwork is a cosmetic option for Balatro’s playing cards. Nintendo describes the collaboration theme as “Bible Thump” and connects it with The Binding of Isaac: Repentance. Its overview describes pixel-art characters on customizable decks.

The official descriptions identify card art, not a gameplay expansion: they do not say the crossover adds new cards with different effects, mechanics or rules. They also do not present the theme as a separate paid add-on.
How to get the Isaac card skins
The skins were included in Friends of Jimbo 2, which Balatro’s official Steam announcement described as completely free. The announcement was dated October 24, 2024, and said the update was available on Steam, consoles and mobile at launch. Read the official Balatro announcement on Steam.

The available official descriptions do not document current, platform-specific menu steps for selecting the skins, so the exact interface may depend on your installed version. They also do not say that owning The Binding of Isaac: Repentance unlocks the Balatro cosmetics; it is identified as the associated game, not as a purchase requirement.
