It is a reusable travel item that sends Wolf back to a saved location, and it is the game’s name for this mechanic, not “Homeward Bone.”
What the Homeward Idol does
The Homeward Idol returns Wolf to one of two places: the last Sculptor’s Idol he visited, or the Dilapidated Temple. According to the community-edited Sekiro: Shadows Die Twice Wiki entry on Fandom, the item can be used repeatedly, and the TrueAchievements walkthrough for the GOTY Edition’s game basics describes its uses as unlimited. Both references describe it purely as return travel.

Where it can take you

The references establish these two options only. Check the destination prompt when you use the item rather than assuming it always goes to the spot you expect.
Using it safely
The Fandom wiki entry warns that Wolf is completely vulnerable while the item is being used. Treat it as a planned reset, not an emergency escape.

- Do not count on it to break you free from a surrounded fight.
- Do not expect it to restore health or grant a second life. The item references describe return travel only, not healing or resurrection.
Where the “homeward bone” name comes from
“Homeward bone” appears to be a remembered name for a similar item in another game, rather than Sekiro terminology. Player discussion on the r/Sekiro subreddit uses informal wording such as “use homeward idol to tp back to the last statue/checkpoint,” which shows how players describe the item in everyday speech, while the official in-game name remains Homeward Idol.

The Fandom wiki quotes the item’s description as: “A small, palm-sized wooden Buddha. Can be used repeatedly. Held by the Wolf ever since he was saved by his father, its corners are rounded with use. This Buddha is used to return home.”
