There are 28 Altars of Lilith in Diablo 4’s Fractured Peaks. The map below groups them by the region’s ten named subregions; use its numbered pins to identify exact positions, since the list alone does not provide reliable coordinates or turn-by-turn directions. Activating the set grants permanent, account-wide bonuses, and some altars require Stronghold progress to reach.

The numbered distribution below follows Game8’s regional map. Numbering is specific to that map, so use the pins on the linked map for each altar’s location rather than treating the numbers as in-game coordinates.

Hard-to-spot altars and Stronghold gates
Some pins are not obvious from the road or require clearing a Stronghold. Windows Central highlights these guide-specific examples; check the linked map and your in-game surroundings when navigating:

What the 28 altars reward
Wowhead’s guide, updated March 11, 2026, lists each Fractured Peaks altar as granting either +2 to a stat or +5 to the Murmuring Obol cap. Completing the regional set provides:

Wowhead describes altar bonuses as permanent and account-wide. The same guide says no altar is missable, though access to some may depend on completing relevant content such as a Stronghold. Check Wowhead’s Fractured Peaks altar guide.

For a full sweep, follow the numbered map by subregion and compare its pins with your map’s altar markers. If you are exploring naturally, collect visible altars as you encounter them, then revisit the subregions with remaining pins. Before making a detour to a gated location, clear the associated Stronghold; the map shows where to look, while the in-game map helps you track which pins remain.
