Borderlands 4 has three documented base-game Primordial Vaults in Kairos: Arch of Inceptus in The Fadefields, Arch of Radix in Carcadia Burn, and Arch of Origo in Terminus Range. Each requires the three Vault Fragments found in its own region. The key entrance clues are a hilltop, a desert roadside, and a mountain tunnel reached by gliding across a gap.

PowerPyx’s September 11, 2025 location guide lists these three Vaults, one in each of the three regions above, and says Dominion has no Vault. The regional map guides also identify the corresponding Vault locations and three-fragment requirements: The Fadefields, Carcadia Burn, and Terminus Range.

How to reach each entrance
Arch of Inceptus: The Fadefields
Travel to the Dissected Plateau and look for the Vault on top of a hill. The hilltop is the key landmark; the source guide does not provide exact coordinates, so use the subregion name and in-game map labels rather than relying on a coordinate estimate.
Arch of Radix: Carcadia Burn
In the Grindstone of the Worthy, look in the desert sand along a road. The entrance is described as roadside rather than hidden behind a separate interior route.

Arch of Origo: Terminus Range
In The Low Leys, find the mountain tunnel. Reaching it requires gliding across a gap, so look for a suitable launch point near the ravine and cross to the tunnel entrance. This is the most route-specific of the three location cues.
Collect the regional fragments before entering
Each Vault uses its own region’s key. Find the three Vault Fragments in that region; after the key is obtained, the Vault icon appears on the map. At the entrance, interact with the pillar and insert the matching key to open a purple portal. Do not mix fragments between regions: the Fadefields key is for Inceptus, the Carcadia Burn key for Radix, and the Terminus Range key for Origo.

Ben-Gun’s September 12, 2025 video guide describes nine Vault Key Fragments in total, three in each of the three regions. It also suggests that fragments appear on the map after capturing a nearby Order Silo. Treat the Silo detail as a secondary guide’s navigation tip, not as a confirmed prerequisite for fragment collection.

- Enter through the purple portal and accept the side quest.
- Take the activated yellow ramp into the arena sequence.
- Clear three arenas of enemies.
- Defeat the Guardian in the fourth arena; the guide reports that this grants a permanent power-up.
The available location information does not establish a reliable comparison of Vault difficulty or a complete, Vault-by-Vault reward list, so these should not be treated as interchangeable measures of which one to attempt first.

This list covers the three documented base-game Vaults in Kairos. Borderlands Support’s March 26, 2026 Story Pack 1 update identifies The Whispering Glacier as a new zone, but does not identify a Primordial Vault there. The documented base-game list therefore should not be expanded to include that area without confirmation of a Vault location.
