Spoilers for Astarion’s Act Three quest, romance, and epilogue. His story branches after the confrontation with Cazador: Astarion can complete the Rite of Profane Ascension, stop and remain a vampire spawn, leave the party, or die if you interrupt a ritual already underway. The imprisoned spawn’s fate is a separate choice, and romance adds further scenes without deciding whether Astarion ascends.
How the choice after Cazador determines Astarion’s fate
After defeating Cazador in the Szarr Palace dungeon, Astarion asks for help completing the Rite of Profane Ascension. Your response and the ritual’s circumstances determine whether he ascends, remains a spawn, or leaves. These are distinct quest outcomes rather than a simple good-or-evil split. The detailed branches below are documented in the community-maintained BG3 Wiki’s The Pale Elf reference; exact dialogue and some variations can depend on party composition and the choices made during the scene.
Help Astarion complete the rite
If the rite succeeds, Astarion becomes a Vampire Ascendant, takes over the palace, and gains status and power in Baldur’s Gate. This is the ascended route. On a romance path, it also leads to a separate decision about whether your character accepts his offer to become his vampire spawn.
Persuade him to stop
If you persuade Astarion not to complete the rite, he kills Cazador but remains a vampire spawn. This is the path that leads to his graveyard scene if you are romancing him. It does not, by itself, decide what happens to the other spawn held in the dungeon.

- Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
- Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
- 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.
Refuse him, fail to convince him, or interrupt the ritual
If Astarion is present and you refuse to help, he destroys Cazador’s staff, leaves the imprisoned spawn behind, and departs the party. If you refuse or fail to convince him to stop, he can also leave. These refusal and failed-persuasion outcomes are not the same as persuading him to remain a spawn.
If you interrupt a ritual already in progress, Astarion attacks and must be killed. That is different from stopping him before the ritual is underway.

When the rite cannot be completed
The rite may become impossible if another spawn is killed during the fight or the required companions are absent. The scene then proceeds as though Astarion abandoned the ritual, though some dialogue differs. Small branch variations are possible, so treat the dialogue and party details as conditional rather than as a guaranteed identical sequence.
What happens to the imprisoned spawn?
Once the rite is stopped, you must separately decide whether to kill the imprisoned spawn, free them, or leave them imprisoned. Freeing them sends the other spawn to live in the Underdark. Astarion’s reaction and the reasoning he expresses vary with this decision; it is not part of the ascension choice.

How romance changes Astarion’s endings
Romancing Astarion does not force him to ascend. The ascension decision and the romance are separate branches, and the ascended romance has an additional choice about your character’s transformation.

Romanced Astarion remains a spawn
If you romance him and persuade him to stop the rite, a graveyard scene follows. Astarion marks a new birth date on his tombstone and asks you to remain his partner. If you encouraged him to search for a way to walk in sunlight, the epilogue says he has been looking for one.
Romanced Astarion ascends
If he ascends while you are in a romance, he may offer to turn your character into his vampire spawn. Accepting is an additional choice, not a requirement for completing his quest or remaining on the romance path. Refusing has its own relationship consequences; the romance reference describes a possible breakup while Astarion remains in the party. See the community-maintained BG3 Wiki romance reference for the scene branches.

What the reunion epilogue can reveal
The reunion-party description reflects Astarion’s route and other choices. Ascended Astarion presents himself as a powerful figure in the city. On the spawn route, freeing the prisoners and proposing that you lead them together can lead to an epilogue in which you and Astarion have built a vampire haven in abandoned ruins in the Underdark, and Szarr Palace has been destroyed. In some circumstances, Astarion can lead the spawn alone instead.
If his personal quest is unfinished and Cazador is still alive at the story’s conclusion, the character reference describes a different outcome: Astarion loses his ability to walk in sunlight and hides from Cazador’s control. That is not the spawn ending reached after the confrontation. The reunion and character outcomes are documented in the BG3 Wiki epilogue reference and Astarion character reference.
What to know about patches and older guides
Larian’s Patch 8 announcement, dated April 15, 2025, identifies it as the final major patch introducing content and says no new narrative content or significant companion-storyline changes are planned. Earlier, Patch 6 notes included fixes to Astarion-specific epilogue reactivity. Older guides may therefore describe behavior that was later corrected. The branch details here come from community-maintained game references, while Larian’s patch notes establish the stated patch status and fixes.
