Baldur’s Gate 3: All Karlach Endings Explained

Karlach can die in Baldur’s Gate, return to Avernus with a possible lead on a heart fix, or become a mind flayer. Here are the tradeoffs in each ending.

Spoilers: This guide covers Karlach’s endgame choices, the Netherbrain finale, and her six-month epilogue.

Karlach has three main engine outcomes in Baldur’s Gate 3: she can stay in Faerûn and die, return to Avernus and survive the immediate finale, or become a mind flayer. Each resolves the threat to her infernal engine at a cost; there is no universally best ending.

How the three endings compare

The right choice depends on what matters most to you: Karlach keeping her familiar self, her wish to remain in Baldur’s Gate, or a chance to continue living. The game does not establish one route as the objectively best ending.

Stay in Faerûn: Karlach dies

If Karlach has not undergone ceremorphosis and is not persuaded to return to Avernus, her engine overheats after the Netherbrain is defeated. She may accept her imminent death in Baldur’s Gate. Karlach says: “It’s the one thing I can’t beat, isn’t it? I wanted to live. In my city. With my friends.”

This ending honors her desire not to return to the Hells and preserves her selfhood, but she dies. In the game’s epilogue, Withers says her soul burns brightly in the Fugue Plane; that is the story’s supernatural account of Karlach, not a general claim about real-world beliefs.

Return to Avernus: survive the finale, face the Hells

Before her engine fails, you can try to persuade Karlach to return to Avernus. The destination means renewed danger rather than a permanent cure, but she survives the immediate ending scene with her engine calmed.

Who can go with Karlach?

You can accompany her. If Wyll became the Blade of Avernus after his personal quest, he can offer to go as well. Larian’s Patch 2 notes describe the available arrangements: you can go with them, go alone with Karlach, or let Wyll go with her by himself.

What the six-month epilogue adds

Larian’s Patch 5 update added a playable epilogue set six months after the main events. In that epilogue, Karlach says she and her companions recently defeated a powerful cambion carrying blueprints for a possible solution to her mechanical heart. They plan to pursue a forge in Avernus, and she mentions an old contact who may help.

The blueprints are a lead, not proof that Karlach’s heart can be repaired. The epilogue offers hope for a possible fix while leaving the outcome unresolved.

Become a mind flayer: a different body and a cost to identity

Karlach can offer to undergo ceremorphosis as part of the plan to defeat the Netherbrain. The transformation removes the infernal engine as the same immediate threat, and she remains in Baldur’s Gate, but she no longer has her tiefling body.

In the account of Karlach’s particular fate, she feeds on dying patients who want to pass on with dignity rather than suffer. She says she still remembers parts of herself and retains familiar expressions, but memories from the brains she consumes gradually erase her original identity. This is the ending’s cost for Karlach; it should not be read as a definitive statement about every mind flayer in Dungeons & Dragons lore.

Which Karlach ending should you choose?

  • Choose staying in Faerûn if honoring Karlach’s wish to remain in Baldur’s Gate and keeping her herself matter more to you than survival. This route ends with her death.
  • Choose Avernus if continued life in her tiefling form matters most, and you accept the danger of returning to the Hells. The six-month epilogue gives her a possible path toward a heart solution, not a confirmed cure.
  • Choose ceremorphosis if you want Karlach to remain in Baldur’s Gate and accept the transformation and the identity loss described in her ending.

Patch context

Larian added a Karlach Avernus-ending scene in Patch 2 and expanded her story moments between acts. Patch 5 added the playable epilogue six months later, which is why descriptions based only on the immediate dockside choice may omit the blueprint lead. The cited updates do not establish the exact current platform patch number.
