In the Grymforge encounter in Act One, use cold damage and stop the Lava Elemental from regenerating with Bone Chill or Arrows of Ilmater. Keep your party out of lava and clear of bubbling hazards, and avoid fire damage: the elemental is immune to it.
Which Lava Elemental is this?
This guide covers the aggressive Lava Elemental that wanders through the lava lake near the Abandoned Refuge in Grymforge. It attacks when approached. Sorcerous Sundries in Act Three contains a separate, non-aggressive display elemental.
Defeating the Grymforge elemental also stops nearby bubbling lava from bursting into fire surfaces. That makes it easier to reach the central island, where a locked Adamantine chest holds the Sentient Amulet. The chest is separate from the elemental encounter.
How to counter its regeneration
Prevent healing
While the elemental is in lava, it rapidly regenerates hit points. Bone Chill prevents its regeneration whether it is standing in lava or not; Arrows of Ilmater are another option identified for this encounter. Apply healing denial early so damage sticks.

Use cold, not fire
The creature reference lists the Lava Elemental as vulnerable to cold and immune to fire. Cold attacks are therefore the clearest damage choice. The same reference lists resistance to piercing and poison, so those damage types are less effective.
Draw it away from lava when possible
If the terrain and your party’s movement allow, lure the elemental onto ground away from the lava and attack while its regeneration is blocked. It can use Create Lava to put lava beneath itself when no lava is nearby, so moving it away is not a dependable substitute for Bone Chill or an Arrow of Ilmater.

Keep the party out of hazards
Do not position characters in lava. Also watch for the bubbling patches around the encounter: they can burst and leave fire surfaces behind until the elemental is defeated. Move characters clear rather than risking damage while waiting for an opening.
How much health does it have?
The BG3 Wiki creature reference, accessed in 2026, lists these hit-point values by difficulty. They are reference-page game stats, not independently measured values or a guarantee for every game version.
