What makes a backpack “best”?
Choose by the job you need the bag to do. Capacity is how much weight it can hold; weight reduction lowers the encumbrance contribution of its contents while the bag is equipped. Empty weight and the run-speed modifier also matter, especially when a pack is full.

The Build 42.20.4 comparison guide uses this estimate for a full, equipped bag: empty bag weight + capacity × (1 − reduction/100). It is a way to compare the listed properties, not a promise that every situation in-game will produce the same result.

A worn pack helps reduce the encumbrance of its contents. A bag merely carried in ordinary inventory does not apply the same reduction, according to the Build 42 carrying guide. This is about item encumbrance, not the character’s body-weight state.

For reducing the load on a loot run
The ALICE Military Backpack is the pick if weight reduction is your priority: it has the highest listed reduction here at 85%. Its 0.94× run multiplier is also the lowest in this comparison, so weigh that movement trade-off against the benefit of reducing carried contents.

For carrying the most
The Large Framepack has the greatest listed capacity at 35. That extra room comes with 75% reduction and an empty weight of 4.0 in the cited table, so it is not automatically the lightest choice when loaded.
For an everyday balance
The Big Hiking Backpack (22 capacity, 80% reduction) and Hiking Backpack (20 capacity, 75% reduction) trade some capacity and reduction for lower empty weights than the Large Framepack. The 42.20.4 list gives the big version the higher capacity and reduction; extracted B42 item data also lists the standard Hiking Backpack at 20 capacity, 75% reduction, 0.96× run speed, and 1.2 weight.

Do not choose by appearance alone
A military-skinned Duffel Bag remains a duffel in the cited table: 18 capacity and 65% reduction, versus 28 and 85% for the ALICE Military Backpack. Compare the item’s actual stats rather than its color or military styling.

How current are these stats?
The Indie Stone’s official site currently reports Stable Build 42.21. Its release notice says, “Project Zomboid version 42.20 has been released to the public Stable branch!” The specific item tables above are labeled Build 42.19 or Build 42.20.4, so they should not be treated as verified 42.21 stats. The 42.21.0 API documentation defines WeightReduction as a percentage reduction of contained weight, but does not establish current values for individual vanilla bags.
