There is no single best setup for DOOM: The Dark Ages. Tune the setting that addresses your specific friction point: a longer Parry Window for timing, Enhanced Targeting for aiming, combat modifiers for pressure or damage, and accessibility options for contrast, motion, text, audio cues, and controls. Most difficulty presets and modifiers can be adjusted during play, but Pandemonium and Ultra-Nightmare have permanent-death rules and cannot be changed after you start.
DOOM: The Dark Ages combat settings: start with the problem
Parry Window, Enhanced Targeting, and Empowered Attacks solve different problems; none is a substitute for every other difficulty adjustment. Bethesda’s gameplay modifier overview explains what each combat control changes. Use the smallest adjustment that makes combat manageable, then revisit it if the result still does not fit.
Parries are too strict
Raise Parry Window to give yourself more time to perform a parry. Bethesda explains, “At higher values, players have a longer (and thus more forgiving) window to perform parries.” This is the most direct adjustment when the timing itself is the obstacle.
Aiming takes too much precision
Enable Enhanced Targeting for auto-aim assistance that guides attacks toward targets. It can help if fine aim is uncomfortable or difficult; it does not address parry timing or incoming damage.

Enemies interrupt or overwhelm you
Try Empowered Attacks if you want it to be easier to interrupt and stagger enemies that are attacking. If staying alive is still the issue, adjust the relevant pressure or damage modifier instead of expecting Empowered Attacks to solve it.
You want to change how dangerous combat feels
The modifiers let you tune several factors independently. Choose according to the effect you want rather than changing every slider at once.

| Game Speed | Overall game pace | Adjust it if the pace itself is uncomfortable; do not assume changing it has no side effects. |
| Enemy Projectile Speed | The speed of enemy projectiles | Adjust it if projectile timing or tracking is a particular obstacle. |
| Daze | A separate combat modifier listed in Bethesda’s overview | Consider it when tuning combat, and check its in-game description for the current effect. |
The official overview distinguishes these controls, but the available source description does not establish a more specific effect for Daze. Check its description in the game rather than assuming what it changes.

You need a difficulty baseline
Pick the regular difficulty preset that matches the challenge you want, then layer in targeted assistance if needed. Bethesda’s May 2025 overview recommends Ultra-Violence for experienced DOOM players; that is publisher guidance, not a universal best choice. Regular presets can be changed during play. Pandemonium and Ultra-Nightmare instead use permanent-death rules, and their difficulty cannot be changed after starting.

Update 3 added Brink of Death choices. The November 18, 2025 Update 3 notes characterize Normal as the recommended support and Hard as less support. Check the in-game description to choose the option that fits your preferred level of assistance.
Accessibility settings by need
Bethesda’s Accessibility Guide, dated May 13, 2025, describes visual, motion, text, control, and audio-related options. Names and availability can vary with updates and context; consult the current in-game menus, especially when playing the Revelations expansion.

Enemies blend into the background
Use World Desaturation to mute the world, then adjust the high-contrast highlights to make important elements stand out. Bethesda describes these options together as high-contrast mode. Threat Detection can also show visual indicators for critical enemy attacks coming from off-screen.
Screen movement is uncomfortable
Try disabling Weapon Bob and Environmental Screen Shake. Bethesda says disabling either may reduce motion-related discomfort. Change one at a time if you want to tell which setting helps.

Interface text is difficult to read
Adjust the available text sizes for the UI, HUD, and subtitles, and use Magnify where helpful. Revelations Update 4 made subtitle scaling independent of HUD scale, so subtitle size can be adjusted without relying on the HUD setting. The July 7, 2026 Update 4 notes document that change.

You need more comfortable controls
Remap controls to suit your hands and preferred layout. Update 3 added bindable next/previous melee weapon and Shield Rune controls. If opening a weapon wheel during a fight is difficult, direct bindings for these actions may reduce reliance on navigating it.
You rely on audio cues
The guide documents visualizations for combat-related sound cues, including visual indicators for critical off-screen enemy attacks when Threat Detection is enabled. It also states that the game does not support screen-reader menu narration or audio-based navigation assistance. Do not expect those features to be available as accessibility settings.

- Revelations Update 4 — July 7, 2026: added an expansion-specific Perfect Dodge Window, made subtitle scaling independent of HUD scale, and adjusted UI and settings. Its notes describe the damage-direction indicator as a general setting that is off by default.
Perfect Dodge Window is specific to Revelations; do not confuse it with the general Parry Window modifier. For the latest labels and availability, check the current game menus and the relevant Update 4 notes.

A quick way to tune your setup
- Identify the obstacle: timing, aiming, enemy pressure, damage, pace, visibility, motion, reading, or input.
- Change the matching setting: for example, Parry Window for timing, Enhanced Targeting for aim, or text scaling for readability.
- Try an encounter and reassess: if the problem remains, adjust another relevant control rather than assuming one preset covers every need.
- Before starting a permanent-death run, decide on the challenge: Pandemonium and Ultra-Nightmare cannot be changed after the run begins.
