A broken wrist and a sprained wrist can both cause pain, swelling, bruising and difficulty moving the joint. A fracture breaks a bone; a sprain injures the ligaments or capsule supporting the wrist. Symptoms overlap too much to tell the difference reliably at home. If a break is possible, get prompt medical assessment; numbness, an open injury, heavy bleeding, an obvious deformity or signs of poor circulation need emergency care.
What is the difference between a broken wrist and a sprain?
A wrist fracture is a break or crack in one or more wrist bones. It may involve the radius near the wrist or one of the small carpal bones. A fall onto an outstretched hand is a common cause; the distal radius is one of the most commonly fractured wrist bones, according to Mayo Clinic.
A wrist sprain is an injury to the ligaments or joint capsule that support the wrist. Falls can cause either injury. The difference is the tissue affected, not a symptom pattern that can be confidently identified without an assessment. See Mayo Clinic’s wrist sprain overview.
Symptoms: why you cannot reliably tell at home
Both injuries can cause pain, tenderness, swelling, bruising, stiffness and difficulty moving the wrist or gripping things. A sprain may hurt more with wrist movement, gripping or twisting. A fracture may also cause severe pain, stiffness, difficulty moving the fingers or thumb, or numbness. These clues do not reliably separate the injuries: being able to move the wrist, having little bruising, or lacking an obvious deformity does not rule out a fracture.

The NHS notes that it can be hard to tell whether a wrist is broken, dislocated or badly sprained. A clinical examination and often an X-ray are needed to check for a break. NHS guidance on a broken arm or wrist and its wrist-pain guidance describe symptoms that can overlap.

- Numbness, tingling or pins and needles in the hand or fingers.
- Bone visible through the skin, a heavily bleeding cut, or another open injury.
- The wrist or arm is at an abnormal angle or visibly deformed.
- Fingers or skin change color, or there are other signs that blood flow may be impaired.
Call your local emergency number or use your area’s emergency service. The NHS advises UK readers to go to A&E or call 999 for the listed urgent signs; those service names and numbers are not universal. See NHS emergency guidance and Mayo Clinic’s broken-wrist guidance.
When to arrange prompt medical assessment
Seek medical advice as soon as possible if you suspect a fracture, the pain is severe or worsening, you cannot use the wrist, swelling or bruising is substantial or increasing, or movement is very limited. A suspected break should not be managed by waiting to see whether it improves: delayed diagnosis and treatment can lead to poor healing, reduced range of motion and weaker grip strength.

In the UK, NHS 111 is one route for urgent advice when emergency signs are absent. Elsewhere, use the local urgent-care service. For wrist pain that interferes with normal activities, worsens or returns, or has not improved after two weeks of home care, the NHS advises contacting a GP. Do not wait two weeks if a fracture or emergency signs are possible. See NHS wrist-pain advice.
What to do while waiting for assessment
- Support the arm. Keep it as still and comfortable as possible; a towel can be used as a sling.
- Apply a wrapped cold pack. The NHS advises applying one gently for up to 20 minutes every 2 to 3 hours. Do not put ice directly on the skin.
- Remove rings and watches. Do this before swelling makes them difficult to remove.
- If there is bleeding, cover it. Use a clean pad or dressing if possible.
- Follow local emergency guidance. The NHS advises not driving yourself to A&E; arrange help or transport according to the situation.
These are interim measures, not a way to diagnose or treat a fracture at home. Refer to NHS advice for a suspected broken arm or wrist.

How clinicians diagnose and treat the injury
A clinician may ask how the injury happened, examine the wrist and arrange an X-ray. X-rays can help identify a fracture; they may also be used when assessing a sprain to rule out a break, joint instability or arthritis. Depending on the concern, MRI or ultrasound may be considered to assess deeper joint structures or tendons. Details of sprain assessment are available from Mayo Clinic.
Treatment depends on what the assessment finds and how severe the injury is. A fracture may need to be realigned and immobilized in a cast or splint. A sprain may be protected with a splint while it heals. The appropriate support and care plan depend on the specific injury, so a brace or splint should not be used to decide whether the wrist is broken.
