The player must have been designated before kickoff and meet the league’s roster requirements; simply being the team’s third-string quarterback is not enough.
When can the emergency third quarterback enter?
The NFL’s September 4, 2023 rule explanation says the emergency quarterback may enter only if the club’s two active quarterbacks cannot play because of injury or disqualification. For example, the emergency player may be needed if QB1 is unable to play while QB2 is being evaluated for an injury.
A coach cannot activate the emergency quarterback just because a quarterback is playing poorly or the coach wants to remove him for performance or conduct reasons. The league described that boundary when owners approved the procedure in 2023.
Who qualifies as the emergency quarterback?
“Third-string quarterback” is shorthand, not automatic game eligibility. Before the game, the club must designate a bona fide quarterback from its 53-player roster as the emergency third quarterback. The starting and backup quarterbacks must also be on the 53-player roster, and the club must have two bona fide quarterbacks on its game-day active list.

- A team with three or more quarterbacks on its game-day active list cannot use this special emergency procedure.
- An elevated practice-squad player cannot sit in the emergency role under the rule described by the NFL.
These requirements come from the NFL’s emergency-quarterback FAQ and its 2023 announcement.
Can QB1 or QB2 return after the emergency quarterback enters?
Yes. The NFL says that if QB1 or QB2 is evaluated for an injury and cleared to return, either may re-enter the game. The emergency designation does not permanently remove an earlier quarterback who has been medically cleared.

Does a practice-squad elevation count?
No. A practice-squad elevation and the emergency designation are separate roster mechanisms. In 2024, the NFL reported that the NFLPA vetoed a proposed change that would have let teams elevate a bona fide quarterback from the practice squad an unlimited number of times for emergency duty; the rule reverted to its 2023 parameters. An elevated player joins the active/inactive list for that game rather than serving as the inactive emergency quarterback. See the NFL’s 2024 report on the veto.

The rule returned in 2023 after San Francisco lost its usable quarterbacks to injuries in the NFC Championship Game against Philadelphia. The NFL reported that coach Kyle Shanahan and general manager John Lynch supported an emergency option following that game. The rule provides a narrow fallback for injury or disqualification, not a way to make a strategic mid-game quarterback substitution.
