Nightmare Sigils unlock Nightmare Dungeons. Complete Nightmare Dungeons for a chance at another sigil; if one does not drop, Patch 2.5 says Sigil Powder drops instead. You can also activate another sigil from inside a completed Nightmare Dungeon, so you can continue to the next run without exiting first.
How do you get Nightmare Sigils in Diablo 4?
Nightmare Dungeon completions can reward another Nightmare Sigil, but the drop is not guaranteed. In its Patch 2.5 notes, Blizzard said the chance of receiving another sigil was reduced; when a sigil does not drop, Sigil Powder drops instead. This describes the change in that patch, not a complete list of every current sigil source.

How do you use a Nightmare Sigil?
Activate a sigil to access the Nightmare Dungeon associated with it. Blizzard’s Season 9 announcement specifically confirms that, after completing a Nightmare Dungeon, you can activate another Nightmare Sigil from inside the dungeon. That lets you chain another run without leaving first. Blizzard’s announcement does not specify current interface controls, so the exact interaction may depend on the game’s current UI.
Can you chain Nightmare Dungeons without leaving?
Yes. After finishing a Nightmare Dungeon, activate another sigil from inside to start the next run. This in-dungeon option was documented as part of the permanent Nightmare Dungeon updates announced in Season 9.

What are Escalation Sigils?
Escalation Sigils begin an escalating Nightmare chain. Blizzard’s 2.3.0 PTR overview described them as rewards from Horadric caches granted by Horadric Strongrooms, starting in Torment I. That was a PTR description, not confirmation of the current live source or difficulty requirement. Season 9 also introduced Horadric Strongrooms, Escalating Nightmares, and new Nightmare Dungeon affixes; those are Season 9 additions, not a complete catalog of current features.
