The developer discussion of playing your way and the official game overview frame those systems as central ways to make Sanctuary your own.

- Skills and build: Select skills and develop a direction that suits how you want to play.
- Gear: Equipment and upgrades influence your character’s capabilities and build identity.
- Progression: Decide how to pursue campaign and endgame activities, and whether to continue a persistent character or begin fresh for a season.
These are substantial gameplay choices, but they are not the same as choosing the course or ending of a branching narrative.
Do your choices change the story or ending?
Blizzard describes the campaign as nonlinear, but that description alone does not show that players can choose different major story outcomes. The cited official material does not document dialogue branches, explain which player decisions change quests or character outcomes, or establish alternate campaign endings. It is safest to say that the campaign is presented as nonlinear while the clearest documented player agency lies in character building and progression.

Some consequential decisions belong to characters in the story, not to the player. For example, Blizzard’s Vessel of Hatred page describes Neyrelle’s choice to imprison Mephisto; that is part of the narrative, not evidence that the player controls that decision.
Which realm should you choose?
Blizzard distinguishes Seasonal and Eternal play by how a character’s progression works and what content is available. The realms share core activities such as campaigns, classes, dungeons, and endgame challenges, but they serve different preferences. The official realm overview says players who have completed the campaign can go directly into seasonal endgame rather than replaying the campaign.

Can you change your build later?
Blizzard’s December 2020 developer update said that players would be able to respec skills and passives, while noting that the effort and cost would increase as a character grew. That is historical design context, not a reliable guide to current respec costs or rules. The practical takeaway is that build experimentation was part of the stated design, but consult current in-game information for the live system.

Do expansions add more choices?
Yes, expansions introduce additional options and systems, but those features should not be mistaken for base-game content or proof of branching story endings. Blizzard’s Vessel of Hatred page describes a campaign continuation, the Nahantu region, the Spiritborn class, and the Dark Citadel co-op endgame activity. Its Lord of Hatred page describes skill-tree changes and variants, crafting and item systems, and endgame selection through War Plans, as well as Echoing Hatred. Check the relevant expansion details for availability and ownership requirements; Blizzard’s overview identifies expansion editions separately and says expansions require the base game.
