Its rating summary describes a female character’s exposed buttocks in a hot spring and says players can buy and consume sake, which blurs the screen, and take part in a drinking game. The ESRB does not identify the hot-spring character as protagonist Atsu.
What does the ESRB rating say about nudity?
The ESRB’s official rating for Ghost of Yōtei Complete Edition is M for Mature 17+. Its descriptors include Partial Nudity, alongside Blood and Gore, Drug Reference, Intense Violence, Language, and Use of Alcohol. The rating applies to the Complete Edition identified as a PlayStation 5 title.

In its content summary, the ESRB says: “During the course of the game, a female character’s exposed buttocks is depicted in a hot spring.” This is the specific nudity described in the rating. The summary does not describe explicit sexual activity.
Is the character in the hot spring Atsu?
The official rating does not name the female character in the hot-spring scene. PlayStation identifies Atsu as the protagonist, but that does not establish that Atsu is the character the ESRB describes. Treating them as the same person would go beyond what these official sources confirm.
