Yes, a physical copy of DOOM: The Dark Ages still requires an internet connection and downloads for installation and some content. Bethesda confirms that Xbox and PlayStation physical editions include a disc for the base game, but its public guidance does not say how much game data is on the disc or give the exact download size for disc owners. The listed console storage figures are install sizes, not physical-edition download estimates.
What comes on the physical edition?
Bethesda says Xbox and PlayStation physical copies include a disc for the base game and do not include a base-game code. That confirms there is a game disc, but not how much of the game is stored on it. Bethesda’s official pages do not establish that the disc contains the complete game data, nor do they quantify how much a disc owner must download.

For PC, Bethesda says a Steam “physical copy” is a code to redeem on Steam, not a disc. Platform details matter: Xbox Series X can use the Xbox disc, while Xbox Series S cannot play physical copies. A PS5 Digital Edition can use a physical copy only if it has the compatible disc-drive attachment. Bethesda’s physical-copy and compatibility guidance covers these distinctions.

Why does a disc owner still need internet?
Bethesda’s Collector’s Bundle terms state that “All versions require an Internet connection for downloaded components.” The terms also say internet access and sufficient storage are required to download and install the game and certain content, including mandatory updates. In practical terms, owning the disc does not remove the need to connect and complete the required downloads and installation.

The publisher does not specify whether a disc owner downloads the entire base game, only updates and other components, or a particular amount of data. So the accurate answer is that downloads are required, but the exact download size for a physical copy is not stated. Bethesda’s Collector’s Bundle terms provide the internet and storage requirement.
How much storage should you plan for?
Bethesda Support’s approximate launch install-size estimates were roughly 80 GB on Xbox and 82 GB on PlayStation for the base game. The Premium Edition content was listed at roughly 17 GB. These are install-size figures, not measurements of the download required by someone installing from a disc; Bethesda notes that sizes may vary.

Check that your console has enough free storage for the installation and updates, and allow for variation rather than treating these launch estimates as an exact current download total. Bethesda Support’s console storage guidance lists the approximate figures.
