Expect some room to look around, but a directed crime narrative—not Mafia III-style freedom to roam.
How open is Mafia: The Old Country?
In an announcement dated May 8, 2025, publisher 2K called Mafia: The Old Country a “focused, linear experience.” Game Director Alex Cox gave the design rationale in an Epic Games / Unreal Engine interview: “With Mafia: The Old Country, we want to deliver the kind of narrative experience that feels distinctly Mafia, and creating a more linear, story-focused game was how we could best accomplish that.” 2K’s announcement and Cox’s interview make the intended direction clear: this is a story-first action-adventure, not a sprawling sandbox.

That does not mean the world is only a sequence of sealed corridors. In a July 31, 2025 interview with Xbox Wire, Cox said, “There is a world to explore, but it’s a backdrop to the story, it’s not the focus. It’s not an open world game in a conventional sense.” The Xbox Wire author, writing after playing three hours, reported that players could explore between missions. Why the Mafia III comparison matters
The contrast is about structure and freedom, not a verified map-size comparison. In its October 7, 2016 announcement, 2K described Mafia III as set in the “immersive open world” of New Bordeaux and called it the franchise’s first “true open world.” The Old Country, by comparison, is explicitly framed as linear and story-led. 2K’s Mafia III announcement establishes the publisher’s distinction between the games.

Cox said the team looked to Mafia: Definitive Edition and Mafia II for The Old Country, rather than following Mafia III’s open-world direction. The practical expectation is a world players move through in service of the narrative, with exploration where it fits—not a promise of broad, independent activity across a sandbox.
What the focused structure is built around
Enzo’s rise in early 1900s Sicily
The story follows Enzo Favara as he joins Don Torrisi’s criminal family in early 1900s Sicily. That setting and character journey are central to the game’s identity: the world provides the atmosphere and places for a mob-story progression, rather than serving chiefly as a playground detached from the plot.

Action and period traversal
2K’s announcement identifies melee combat, gunplay, period vehicles, and horseback traversal as part of the experience. These elements offer different ways to engage with the setting, but they do not change the game’s stated linear, narrative-led structure.

The Old Country is a better fit for players seeking a directed Mafia story, cinematic pacing, and exploration tied to missions than for players whose priority is extensive free-roaming freedom. Neither 2K nor the cited interviews establish map area, free-roam duration, mission count, or optional-activity count, so those details should not be inferred from the word “linear.”
