Chained Box: key and code solutions
The Chained Box is in the second-floor lobby. It has three locks: one keyhole, one combination lock, and one keypad. You can enter the two fixed codes without finding their clues; the key still requires a short item route.

Lapis Eye Key
- On the third floor, enter Exam Room 4 and take the Instrument of Force from the medical table beside the dummy.
- Break through the wall into Room D2, then use the D1 ladder to descend to the Kitchen. Continue through the Cafeteria, Storage, and Medical Records.
- In the small room northwest of Medical Records, take the Key of Bliss from the hole in the wall.
- Return by ladder to Exam Room 4. Use the Instrument of Force on the dummy’s right arm, then use the Key of Bliss at its eyehole to obtain the Lapis Eye Key.
Left combination lock: 1622
Enter 1622. For the clue, reach Room M2 by going through the second-floor southern Restroom to L1, then breaking through the wall from M3. The Calendar Page memo beneath the hospital bed encodes the digits as one year, six pills, two hours, and two light flashes.
Right keypad: 9659
Enter 9659. To reveal the code in Room I7, access the third floor through the Director’s Office ladder, follow the red cable to the generator, and power it. Return to the treatment stretcher and use the adjacent machine; it displays the keypad code.

Open the box
Return to the second-floor lobby and use the Lapis Eye Key, 1622, and 9659 on their matching locks. Opening the box triggers a cutscene and gives James the elevator button.
Optional Pharmacy Decorative Box: 2-3-1
In the Pharmacy, enter the elevator quiz answers in this order: 2, then 3, then 1. The box is optional and its reward is supplies. Its answer sequence is also reported to remain the same across riddle difficulties.
Continue through Otherworld Brookhaven Hospital
After opening the Chained Box, the chapter continues through the basement and the hospital’s exit route. Follow these steps to reach the end:
- Insert the elevator button and take the elevator to the basement.
- In the Pump Room, take the Copper Ring from a crate; Maria rejoins James.
- Go to the first floor, enter the Pool Room, pass through the shower area, and visit the Doctors’ Lounge. Take the Lead Ring from the fallen freezer.
- Return to the second floor and use both rings at the Lady of the Door.
- Go downstairs and run through the chase to the elevator. After the cutscene, visit Reception and take the Hospital Entrance Key from atop the book on the desk, then leave.
First-floor starting item
At the start of the Otherworld hospital section, go through the double doors into the Dayroom and take Memo #38, “Evaluation Card, Patient #3141,” from the wheelchair. A save point is at the end of the corridor before you take the elevator to the second floor.
