Tasks are divided into Main Tasks, Side Tasks, and Contracts. Healing has changed too, but the official 0.4 notes do not provide a complete symptom-by-symptom treatment guide. This guide separates what is established from what was still upcoming as of October 8, 2026.
Use the in-game map to find and accept tasks
MADFINGER Games redesigned task progression for Spearhead. The map displays available tasks and approximate objective locations, and lets you accept tasks directly. “Approximate” is important: a marker helps you narrow the search, but does not promise to identify the exact item, interaction point, or route you need.
The developer describes the flow this way: “All available tasks are now shown on the map, including the approximate locations of the objectives, and can be accepted directly from the map instead of through the task list.” Source
Tasks are unlocked through progression across map regions, so an area or task you cannot access yet may require more progress rather than a different map marker. The official homepage also points players to a community-powered tactical map, but the available official information does not establish which external map is most accurate after the redesign. Treat third-party maps as community-maintained references and check their update date.

Know the three task categories
Main Tasks
Main Tasks carry the main story progression.
Side Tasks
Side Tasks provide additional lore about Lamang.
Contracts
Contracts are simpler objectives. The Spearhead redesign introduced a broader progression loop, and MADFINGER reported more than 250 total tasks after adding 100+ new tasks and contracts and updating 50+ tasks in its March 18, 2026 devlog. That is a dated developer figure, not a guaranteed live count.
Plan for task failure and replay rules
MADFINGER says most tasks can be replayed, with Starter Town tasks as an exception. Death can cause task failure; in most cases, the failed task is temporarily unavailable before another attempt. A failure also costs reputation equal to 10% of the reputation success would have awarded. The developer’s example is a 200-reputation success reward and a 20-point failure penalty, not a fixed penalty for every task.
Because task prerequisites and objective details can change, confirm them on the current in-game map rather than relying on an older walkthrough. IGN’s starter-task video dates to May 3, 2024, before the 2026 task redesign, so its directions may not match the current flow.
What the current official notes say about healing
The official Update 0.4 launch changelog reports new medical items, rebalanced symptoms, an improved injury-severity indicator, and an improved indicator for healing other players. Those changes establish that healing mechanics were revised; they do not, on their own, provide a complete treatment order for every symptom or injury.

As of October 8, 2026, Rogue Ops (Update 0.5) had been announced but had not reached its stated full release date of October 22, 2026. An October 4 preview said the health system would be streamlined and selected healing animations made faster. These were previewed changes, not current instructions at the time. Recheck the live notes before relying on them after release.
