How Long Does a Hamstring Strain Take to Heal?

Hamstring strain recovery ranges from a few days for a mild injury to weeks or months for severe tears. Pain relief alone does not mean you are ready to return to sport.

A mild hamstring strain may improve within a few days; a more substantial strain can cause symptoms for weeks or around a month, and severe injuries may take weeks or months. Recovery after surgery for a tendon tear can take several months. Those are broad ranges, not personal deadlines: pain easing is not the same as being ready to sprint, train or compete.

Typical recovery times by injury severity

Hamstring strains range from mild overstretching to a complete tear. MedlinePlus describes grade 1 as mild, grade 2 as a partial tear and grade 3 as a complete tear. It notes that grade 1 symptoms may last two to five days, while symptoms from grade 2 or 3 injuries may last up to a few weeks or a month. These figures describe symptoms, not a guaranteed date for full strength or return to sport.

Mild, grade 1 strain May heal in a few days; symptoms may last two to five days (MedlinePlus, reviewed June 27, 2026). Feeling better does not automatically mean the hamstring is ready for sprinting or sport.
Grade 2 or 3 symptoms Symptoms may last up to a few weeks or a month (MedlinePlus, reviewed June 27, 2026). This is a symptom estimate, not a full-recovery or clearance deadline.
Severe injury May take weeks or months to heal (NHS). A major tear may need specialist assessment and can take substantially longer than a mild strain.
Tendon repair surgery Recovery can take several months (NHS). A suspected tendon injury needs clinical assessment rather than home care alone.
Return-to-play study estimates A 2017 systematic review of nine studies and 601 acute injuries found mean return-to-play times of 12–25 days in studies that used isokinetic dynamometry in return decisions. This study-level range reflects particular participants and methods; it is not a forecast for an individual.

Why “healed” and “recovered” can mean different things

Someone may mean that pain has eased, walking is comfortable, rehabilitation is complete, full training is possible or competition is safe. Those milestones are not interchangeable. Reviews of hamstring injuries have found inconsistent return-to-play definitions and criteria, so timelines based on different outcomes cannot be compared as though they measure the same thing.

The 12–25-day mean reported in the 2017 systematic review applies only to studies using isokinetic dynamometry as part of return decisions. Other studies used different approaches and reported different estimates and reinjury rates. It should not be treated as a standard recovery period for every person or sport.

When it is reasonable to return to running or sport

There is no universally validated calendar or single test that clears everyone to return. A 2016 systematic review found varied return-to-play criteria, including absence of pain, comparable strength and flexibility, functional performance and medical clearance. A worldwide football consensus also includes mental readiness for full training or match availability. That consensus is specific to football and does not prove that any one criterion guarantees safety for every athlete.

In practice, return decisions should consider how the hamstring feels during examination and testing, strength and flexibility, sport-specific function and clinical advice—not just whether pain has subsided. When an injury is significant, recovery is delayed or the planned activity demands sprinting or rapid changes of direction, a clinician or physiotherapist can help guide a gradual return.

What to do during the first few days

For the first two to three days, NHS guidance recommends protecting the leg and resting from activities that aggravate the injury, applying ice wrapped in a cloth, using compression and elevating the leg. Avoid heat, massage and alcohol during the first three days. When movement is no longer stopped by pain, gentle movement can help prevent stiffness.

MedlinePlus also advises resting from painful activity, using wrapped ice, compression and elevation, then increasing activity gradually and following exercises recommended by a health care provider. Crutches may be needed if bearing weight is difficult. These measures are for care and support; the cited guidance does not establish that a compression wrap speeds tissue healing.

When to seek medical advice

Get clinical advice if pain is very severe or worsening, swelling or bruising is large or increasing, standing or walking is difficult, movement is markedly limited, or the injury is not improving with self-care. Sudden numbness or tingling, a sudden increase in pain or swelling, or an injury that does not appear to be healing as expected are also reasons to contact a provider.

A pop, pronounced bruising or pain close to the buttock or knee can point to a more serious injury. MedlinePlus notes that an injury near either end of the hamstring, especially with substantial bruising, may involve a tendon pulled off the bone. An orthopedic or sports medicine referral may be needed, and some tendon injuries require surgery.
