These are estimates, not guaranteed play times.
How long does each playthrough take?
GOG’s listing for the 2014 Tango Gameworks game displays the following HowLongToBeat figures. The page was accessed on October 8, 2026; it does not state when the estimates were last updated or how many player reports they represent.

For the usual meaning of “beat the game”—reach the end of the story—15 hours is the most useful planning figure. The other estimates describe different play goals, not extra time every player must spend. GOG’s listing does not provide a difficulty-specific breakdown.
Does difficulty affect how long it takes?
It can affect the pace and challenge, but the available estimates do not give separate completion times for each difficulty. The game initially offers Casual and Survival; Bethesda says beating Survival unlocks Nightmare and Akumu. Nightmare and Akumu are harder and use different enemy layouts, and in Akumu Sebastian dies from one hit. That makes a longer run plausible, but there is no supported number of extra hours to assign to either mode. (Bethesda on Akumu; Bethesda on difficulty settings.)

The PlayStation 4 manual says Casual is recommended for first-time survival-horror players, while Survival is intended for experienced players. It also says lowering the difficulty during a playthrough restarts the current chapter, so changing settings can cost progress.

After completing Chapter 15, a completed save enables New Game+ and access to all chapters. Starting at Chapter 1 in New Game+ carries over that save’s skill upgrades and completion bonuses, but the available difficulty is limited to settings already completed or lower. The published time estimates do not separately measure New Game+ or chapter replays, so they should not be treated as a timed guide to those runs. These details are documented in the PS4 manual.
