The limit applies to compatible mods for that specific weapon—not any three mods in the game—and some unique weapons do not accept ordinary mods.
How the three-mod limit works
The Fallout: New Vegas PC Manual says each weapon can accommodate “up to three unique modifications.” Mods can change features such as scope zoom, magazine capacity, or damage. The limit is three applicable upgrades, not three copies of the same mod type.

Each mod is made for a particular weapon. For example, the manual notes that a 9mm Pistol sight cannot be attached to a .357 Magnum Revolver. Check that the mod names the weapon you are trying to upgrade.
How to attach a weapon mod
- Open the Pip-Boy and go to the Items list.
- Select the weapon that the mod is designed for.
- Choose the compatible mod to attach it. The weapon is marked with a plus sign after it is modified.
In the unmodded game, an attached mod cannot be removed. Consider whether you want to keep the weapon in its modified form before applying the upgrade.

Why a weapon or mod may not behave as expected
Unique weapons
Most unique weapons do not accept ordinary weapon mods, though there are exceptions. Reference material lists the weathered 10mm pistol and mercenary’s grenade rifle among the exceptions, and the holorifle has its own mod set. Some unique weapons are found with modifications already installed.
Gun Runners’ Arsenal
The Gun Runners’ Arsenal add-on adds weapon mods. The Fallout Wiki lists 40 additional weapon mods and notes that GRA-added mods require the corresponding GRA-labeled weapon variant; the ordinary and GRA versions are not interchangeable for those additions. If a mod appears incompatible, check both the weapon name and its variant.
PC gameplay mods
The three-mod limit describes the vanilla game. WMX documentation describes it as a hard-coded base-game limit, but that does not establish how every other overhaul, patch, or load order behaves. For a modded PC setup, check the documentation for the gameplay mod you have installed.
