Open Telemetry while driving, then cycle to its final page, Damage. It shows damage readings for the car’s listed parts. You do not need external telemetry software to check them.
How to check damage in Telemetry

Get into the car and open Telemetry using its currently mapped control.

Cycle through the telemetry pages until you reach the final page, Damage.

Read the displayed damage percentages for the listed vehicle parts. The fan-maintained Forza Horizon Wiki telemetry reference says each part starts at 0.0% and a warning appears when a part reaches at least 50.0% damage.

If Telemetry will not open
The control used to show Telemetry can depend on the ANNA/Telemetry assignment and platform. Check the game’s current Controls or Advanced Controls mapping and assign the relevant telemetry function if it is missing or unresponsive. A player discussion describes assigning the ANNA/Telemetry switch, but treat that as community guidance rather than an official, platform-wide control instruction; the in-game bindings are the final authority. The January 2025 discussion also reflects players’ informal use of Telemetry to view damage.
In-game Damage page versus external telemetry
The built-in Damage page is the direct way to inspect damage in the game. External UDP telemetry is a separate feature for sending data to compatible software or hardware, and is not needed for this check. MOZA’s Forza Horizon 4 & 5 telemetry configuration guide describes enabling Data Out through Settings > HUD and Gameplay and specifying an IP address and port for an external telemetry consumer.

What to do with an unexpected reading
A December 2023 post on the official Forza Community Forums reports one player seeing tyres at 100% damage after leaving certain player garages with Simulation Damage enabled. It is an individual report, not evidence of a widespread or currently confirmed bug. If a reading looks implausible, recheck it after returning to a garage or otherwise changing the situation; the report does not establish a universal fix.
