Around half health or stamina, he changes attacks: dive underwater to avoid Bullet Bees, and use cover against his Tommy Gun.
Where to fight The Pain
The Pain fight takes place in Chyornaya Peschera Cave. He stays on a rock in the pool, so Snake can use the water to break his line of sight and the rocks as cover. Move between cover rather than standing in the open, and dive underwater regularly to avoid his hornets.

How to handle his hornet armor
When The Pain covers himself with hornets, attacks are blocked until the swarm is scattered or the armor drops. The M37 shotgun is a guide-supported option for dispersing the bees. Game8 search-result excerpts also mention Smoke Grenades and, for non-lethal play, Stun Grenades; treat those as additional options, not a required loadout.

How to deal with grenade-carrying bees
If The Pain’s swarm gathers into a bundle carrying a grenade, shoot the swarm quickly while you are out of the water. The grenade can then fall onto his rock and damage him. The M37 is one guide-recommended way to shoot the bundle. Avoid trying to throw another grenade into the active swarm.
What changes in the second phase
At roughly half of his health or stamina, The Pain unmasks and releases fast Bullet Bees. Dive and stay underwater until they stop circling. If a Bullet Bee hits Snake and lodges in him, open the Cure menu and remove it.

The hornets can also form a Tommy Gun. Use the rocks and water for cover from the barrage, then return to the same pattern: evade, wait for an opening, and attack when the armor is no longer protecting him.

Choose lethal or non-lethal damage

These weapons are useful options, not a verified mandatory equipment list for every difficulty or version. Both approaches rely on the same core tactics: use cover and the water, wait out the bee armor, and attack during an exposed window.
