The tool and category details below are drawn mainly from a third-party guide, not an official symptom encyclopedia. Treat them as practical guidance that may change with updates; the game’s own unlocked symptom list is the reference to follow during play.
How to inspect a survivor for symptoms
- Make a full visual pass. Walk around the survivor and inspect the front, sides, back, under the arms, and feet. Crouch to see low or obscured areas. A flashlight can help distinguish similar marks in dim light.
- Check for behavioral clues. Coughing, sneezing, hiccupping, or holding the stomach may draw attention to a possible issue. These cues come from a fan-made guide and should prompt further examination, not replace it.
- Use the available examination tools. Hidden, internal, or tool-specific findings may not be visible on the skin. Match the tool to the kind of result you need to check, using the reference below.
- Compare results with the current in-game symptom list. The available tools and symptoms expand as you progress. Use the unlocked list rather than assuming every symptom can be identified at the start.
- Record every finding. Select the observed symptoms in the symptom menu before completing the inspection. A Steam Community discussion reports that omitted findings can lower the grade even when the player makes the correct routing decision.
- Choose the destination based on the game’s category and scenario. Steam’s official description says suspicious cases can be sent to quarantine or the lab, while healthy survivors go to the living block. Follow the screening options presented in your current situation.
Which tool detects each kind of finding?
This reference summarizes the tool descriptions in PC Gamer’s January 14, 2026 guide. It is third-party guidance, not a complete official tool manual.

How the game’s symptom categories differ
PC Gamer’s guide groups example findings into three in-game categories. These are fictional game classifications, not real medical guidance, and the guide’s list may not cover every symptom or remain current after updates.

The temperature and heart-rate ranges above are the guide’s reported values, not independently confirmed official specifications. Check your in-game symptom list and readings rather than treating these ranges as medical thresholds.
Why a correct decision can still earn a poor inspection grade
In a January 13 Steam Community discussion titled “Poor Inspections Randomly,” players describe receiving a poor grade despite sending an obviously infected survivor to the lab. A commenter says the manual describes “Poor Inspection” as below 50% of symptoms listed, a regular inspection as above 50%, and an “Excellent inspection” as above 75%.

Those thresholds are community-reported, not an official developer statement, and may have changed. The practical lesson is to record each symptom you can identify instead of relying only on the final routing decision. The same discussion reports that early inspections can be confusing when a symptom is not yet detectable with the tools available to the player; this is player experience, not a confirmed universal rule.
Distinguishing similar marks and avoiding missed findings
Bruise or necrosis?
PC Gamer notes that these marks can look similar in low light. Its guide describes necrosis as darker and cooler-toned, while a red bruise appears deeper purple. Use a flashlight and check the scanner rather than deciding from a quick glance.

Visible clues are not the whole inspection
A surface check can miss internal findings, sounds, eye findings, reactions, or signs revealed by UV light. Work through the tools available to you and inspect concealed areas before you finish.
Symptom categories are game mechanics
Labels such as “Safe,” “Quarantine,” and “Liquidation” describe the game’s screening system. They are not real-world medical classifications or diagnostic advice.
