The Sims 4 pregnancy guide from EA does not document an abortion interaction. The documented ways to end a pregnancy are community mods: Lumpinou’s optional Relationship & Pregnancy Overhaul (RPO) Module 11, or LittleMsSam’s separate Miscarriage Chance & Abortion mod. Choose RPO if you want its preference-linked emotional responses and spouse-accompaniment option. Choose LittleMsSam’s mod if the hospital trip and clearly stated cost and trimester window suit your playthrough.

How to use RPO Module 11
Install the module
- Check Lumpinou’s current RPO download and update information, including the Module 11 post. The post is dated December 16, 2022, so use current creator information to confirm compatibility before installing.
- Remove any previous version of the module. Install the RPO Core Library if you do not already have it.
- Place the Module 11
.packagefile in the Sims 4 Mods folder or one subfolder inside it. Do not extract the file.
Find the interaction in-game
Look under Pregnancy & Family for Go Get an Abortion. The RPO interaction directory also lists Ask to Accompany to Abortion and Ask to Consider Terminating Pregnancy. These interactions are contextual, so they may not appear for every Sim or in every situation. The directory labels itself version 2.986; consult Lumpinou’s current creator information for installation and compatibility details.
How LittleMsSam’s hospital mod works
Install LittleMsSam’s Miscarriage Chance & Abortion mod using the creator’s current instructions, then use its in-game interaction to send the pregnant Sim to the hospital. The documented option costs 1,000 Simoleons and is limited to the first and second trimesters. Its standard moodlet lasts two days; a separate optional add-on changes that duration to seven days.
