Check the entrance for a handle, a spell-marked cube, or another environmental clue; then inspect the room after entering, since opening the entrance may not reveal the chest.

If you see a handle or switch, try Accio. Pulling a switch with Accio is one documented way to open a vault entrance. [Source]

Spell-marked cubes
Move the cube onto its pedestal, then cast the spell indicated by the symbol on the cube. A fire symbol, for example, points to a fire spell. Some cubes need to be lifted or repositioned before they can be used. [Source]
Environmental clues
Not every entrance uses a handle or cube. Reported vault patterns include diving through a whirlpool and lighting torches. Read the surroundings rather than cycling through spells at random. [Source]

Once inside, look for a second puzzle
Getting through the entrance does not guarantee that the chest is immediately reachable. Search for hidden objects, platforms, and mechanisms that change the room.

Watch what changes when you interact with an object: a moved cube, lowered floor, revealed platform, or opened passage can point to the next step.
