For the NHSBT small organ transport box covered by its instructions, record the date and time it was received, then cut the cable tie on one side only of the silver closing mechanisms with scissors. Do not open the sealed service hatch. This is a professional receipt procedure for NORS teams and transplant centres—not general guidance for opening an unidentified container.
Opening the NHSBT small organ transport box
The steps below follow NHS Blood and Transplant’s Small Organ Transport Boxes – Instructions for Use. They apply to the small box described in that document, not every organ transport container. Use your transplant centre’s current controlled instructions and receipt protocol.

- Record receipt. The person taking custody from the transport driver must enter the date and time received on the label. The instructions identify this as mandatory information for the HTA-B form.
- Find the cable tie. Locate the tie on one side of the silver closing mechanisms.
- Cut one side only. Use scissors to cut that tie. The opening instructions do not tell you to cut ties on both sides.
- Open the lid using its closing mechanisms. The lid catches are designed to act as hinges if the lid needs to be opened from one side. Do not pry at or try to open the service hatch.
- Continue with the centre’s receipt process. Follow local procedures for receipt, identification, sample handling and documentation. The opening instructions do not specify how to handle the organ or sterile inner packaging.
NHSBT’s instructions for use cover small boxes used by NORS teams and transplant centres for kidneys, pancreas, blood vessels, hearts retrieved for valves and research tissue.

Do not open the sealed service hatch
The hatch in the lid is sealed. NHSBT explicitly warns: “DO NOT attempt to open the service hatch in the lid. This is sealed shut and attempting to open it could damage the box.” Use the designated closing mechanisms to open the lid; do not treat the hatch as an alternate access point.

Who should open the box?
Organ transport and receipt are controlled professional workflows. The NHSBT instructions are addressed to NORS teams and transplant centres. If you are not part of the receiving transplant team and encounter a sealed organ box, leave it sealed and contact the courier or responsible transplant team. For a different container design, use that container’s own instructions and the responsible team’s protocol.
In the United States, HRSA notes that OPTN members have applicable requirements for organ packaging, labeling, shipping, storage and tracking. Its guidance also advises couriers and organizations to check current airline procedures and applicable FAA and TSA requirements when commercial air travel is involved. Those requirements do not replace the specific opening instructions for the NHSBT box.

Why the procedure is not universal
The one-sided cable-tie instruction is specific to the NHSBT small transport box described in its manual. Other boxes may use different closures or procedures, so do not infer how to open an unidentified container from this example. Opening the outer box also does not establish how to handle the organ or its sterile packaging; those steps belong to the authorized team’s process.

NOTTO guidance likewise describes organ packaging as a medical activity under qualified clinical direction, including preservation-fluid bagging, ice, documentation and secure sealing before transport. It supports leaving preparation and handling to the responsible clinical team rather than treating an organ box like ordinary freight.

The detailed opening steps above are based on the accessible NHSBT instruction document from 2017. NHSBT search results also surfaced a later document, INF1512/2, with an effective date of 11 November 2022, but its PDF was not accessible; teams should follow their current controlled local copy. HRSA’s Organ Transport page was last reviewed in August 2026.
