How to Prevent Ankle Sprains During Basketball

A regular neuromuscular warm-up, targeted balance and strength work, and extra care after a prior sprain are practical ways to reduce basketball ankle-sprain risk. Braces may help; high-tops alone are not proven protection.

The best-supported way to reduce ankle-sprain risk in basketball is to combine a regular neuromuscular warm-up with strength, agility and balance training. Players with a previous sprain should give prevention and rehabilitation extra attention; a brace or tape may add protection, while high-top shoes alone are not proven to prevent sprains.

Why ankle sprains happen in basketball

Many basketball ankle sprains happen on landing. A player may come down on another player’s foot, causing the ankle to roll inward. Because this can happen during ordinary play, prevention is best treated as a routine rather than something to consider only after an injury.

A previous sprain is an important risk factor. In a 2018 pooled analysis of 2,265 Alberta soccer and basketball players aged 11–18, a history of ankle sprain was associated with higher sprain risk (IRR 1.98; 95% CI 1.38–2.81). The analysis also found higher risk among basketball participants than soccer participants (IRR 1.83; 95% CI 1.18–2.85). These are group-level associations, not predictions for an individual player. Build a warm-up around balance, strength and movement

Use a neuromuscular warm-up that includes aerobic activity, strength, agility and balance rather than relying on a few minutes of casual shooting. In the pooled youth analysis, this type of training was associated with lower ankle-sprain risk (IRR 0.68; 95% CI 0.46–0.99). The study included both soccer and basketball, so its estimate is not basketball-only. Owoeye et al., 2018.

For practical planning, include movements that challenge control as well as conditioning: single-leg balance, controlled changes of direction, strengthening around the ankle and lower leg, and jumping and landing practice. The American Academy of Pediatrics recommends preseason ankle strengthening and plyometric exercises for basketball and volleyball players. AAP guidance on basketball and volleyball.

Balance work is especially well supported for preventing another sprain after an injury. The 2021 clinical practice guideline strongly supports balance training for recurrent sprains, while evidence for preventing a player’s first sprain is less established. It does not establish one ideal exercise type or dose. A balance board is optional: balance exercises do not require purchasing equipment. 2021 clinical practice guideline.

Consider a brace or tape, especially after a prior sprain

Bracing or taping can reduce sprain risk in sports participants. A 2018 guideline update reported lower risk of recurrent sprains (RR 0.30; 95% CI 0.21–0.43) and first-time sprains (RR 0.69; 95% CI 0.49–0.96) with brace or tape use. These findings cover sports participants generally, not basketball players alone. The guideline describes the choice as dependent on preference and practical considerations. A brace is a reasonable option to discuss with a healthcare provider, particularly for a player with a prior sprain or another risk factor. The AAP notes that a brace or tape applied by a certified athletic trainer may offer additional support, especially after previous sprains, and advises following a healthcare provider’s recommendation. Comfort, fit and whether the player can use the support consistently are practical considerations; the cited sources do not identify a best model or brand. Choose suitable shoes, but do not rely on high-tops

Wear appropriate footwear for basketball, but do not assume a high-top shoe will prevent an ankle sprain. The ankle-sprain guideline found no difference in protective effect between high-fitted and low-fitted shoes. The sources cited here do not establish a particular shoe model or playing surface as the best way to prevent sprains. British Journal of Sports Medicine guideline update, 2018; AAP guidance.

For youth players, make room for rest

Youth teams should schedule at least one day each week away from organized basketball and avoid high-density competition schedules, in line with NBA youth guidelines. These are broader workload and health recommendations; the guidelines do not quantify them as an ankle-sprain prevention intervention. They apply to youth, not as a universal scheduling rule for adult players. NBA youth health and wellness guidelines.

Returning to play involves more than waiting for pain to settle: the AAP identifies weight-bearing exercise and restoring range of motion, strength and balance as key parts of recovery. A clinician should guide the individual return-to-play decision; the sources cited here do not provide a complete clearance protocol.
