If the Stimpak prompt will not appear, melee-hit the companion until it does, then use the Stimpak. If they remain stuck, dismiss them and fast travel away; Bethesda Support says they will return to their home location, where you can hire them again.
Revive a companion with a Stimpak
- Go to the downed companion and try to use a Stimpak as usual.
- If the companion does not respond and the prompt is missing, hit them with a melee attack until the Stimpak prompt appears.
- Use the Stimpak through the prompt.

If the Stimpak prompt still will not appear
Dismiss the companion, then fast travel to another location. Bethesda says the companion will return to their home location, where you can hire them again. This is the fallback for the stuck-down situation described in its support guidance; it is not a verified recovery procedure for every companion type, DLC setup, platform, or modded save.
What changes in Survival Mode?
Bethesda’s Survival Mode announcement says companions no longer automatically get back up if downed during combat. Do not rely on waiting alone to restore a downed companion in Survival Mode. The announcement does not provide a complete, item-by-item recovery guide for every companion.

Can Dogmeat die?
Bethesda’s September 21, 2015 companion article states: “In Fallout 4, Dogmeat cannot die.” That statement addresses whether Dogmeat can die; it does not cover every stuck state or issues caused by mods. Read Bethesda’s companion article.
Should you use a console command?
Bethesda describes Fallout 4 console commands as primarily a debugging tool and says they are unsupported. Its guidance warns that commands may cause game issues and recommends loading an earlier save if a problem follows command use. For that reason, avoid relying on an unverified revive command as the first fix. Bethesda Support: console command guidance.
